Employee was exiting the maintenance garage when they slid on wet wood at the bottom of the doorway causing their foot to slip out from under them and right knee buckled. Employee wrapped knee with ace bandage but on 1/6/2023 they sought additional first aid receiving a steroid shot and given restrictions, employee off until next doctor appointment.

EE was aiding in measuring dust collector bags when a colleague noticed they were unresponsive to questions. Colleague stated they were unresponsive and was ready to fall and grabbed them. Operator hit head and got cut above eye. 911 was called, EE came to and EMS transported to hospital. Appears a seizure occurred due to de-hydration. 3 stiches were given.

Operator was walking to break room when operator slipped on ice and landed on right arm/elbow. Operator requested additional medical attention. Operator was given a shot for pain and a sling to mobilize the arm. Operator to report to DR in 2-3 for evaluation.

Operator was trying to remove a stuck bolt from a pan grind roller when a splinter of metal chipped off and penetrated the Operators left hand between EE's thumb and pointer finger. Operator had to receive medical attention to remove metal. Local anesthesia and tetanus shot given. No lost time, Operator returned to work.

Operator was engaging a dock level plate when they felt a pulled muscle in their back. Operator left plant to see DR and received a shot for the pain and was told to stay off work for 1 day rest. Operator to return on 1/24/20.

Operator states Operator strained left upper arm while changing a battery for an electric forklift. Interview found Operator provided a false statement on the accident report & did not wear required PPE. Hospital paper work for treatment on 6/19/19 states treatment for left shoulder. Operator received a shot. Claim is suspicious and is under investigation by workers comp.

Operator reported a popping feeling in the left shoulder and bicep while removing plywood from bulk bags. No first aide was required. Since then, operator has had a MRI and received a cortisone shot on 4/29/19. Operator begins PT on 5/1/19 for 4 weeks.

Miner reported a pulled muscle working on a RAL. TQM followed up with Miner @4 P.M. to complete incident report. Miner stated ee took aspirin for discomfort, no further first aide was requested. Miner called off on 5/1 and went to Med-Express, received steroid shots and 800 mg of ibuprofen and a DR note not to work on 5/1/18.Miner did not follow procedure for workers comp.

Operator performed an unsafe act by placing left hand in a machine to remove wet material after SS instructed EE not to because the machine was in start up mode. The Operator put hand into the discharge chute & the machine started up and cut EE's left pinky. First aide was given until an ambulance arrived. Operator was referred to plastic surgeon later that afternoon.

Operator was restacking pallets, when operator backed up forklift, operator did not have left foot completely inside the forklift, which became pinned between the forklift and a concrete pillar. Operator was given basic first aid until an ambulance arrived. Operator received one stitch, and is referred to a foot specialist on 12/4/17.

Operator was exchanging a motor 40lbs. and gear box 50lbs. when they lost grip, lost balance and the item fell and landed on their left hand. Basic first aide was given, employee requested further evaluation. Operator was transported to the hospital at 8:00P.M. Left hand was bandaged. Operator has a fracture and surgery was performed on 10/24/17.

Operator was cleaning the C1 mixer and cut left thumb. Basic first aide was given. Operator requested further medical evaluation. Operator transported to hospital at 5:30PM and received one stitch and a tetanus shot. Operator returned to work at 6:50PM that evening.

On 11/14/16, Operator was picking up a bucket of pellets when they heard a pop noise from right shoulder. No first aid was required. In the coming days no improvement was noted. Operator went to Dr and was given a prescription and shot for discomfort. Operator to follow up 2 weeks from 12/2/16.

Employee observed spillage at a belt conveyor transfer point. Employee twisted his knee as he reached for the emergency stop cord on the plant feed belt conveyor. Employee apparently strained ligaments of right knee. As per doctor report, employee will be able to return to work on 6/15/09.

Employee was walking from break room on level concrete mill floor when knee gave out for no apparent reason (employee did not trip, slip or fall). Employee complained of pain in his knee and was taken to hospital for evaluation. Instructed by hospital physician to take one shift off work. Workmans Compensation has determined this to be a "Life Event" and not an injury.

The employee was using an 8 ft 2x4 to thumb on a bin to loosen a possible bridge over of material. He was holding the end of the 2x4 in his right hand supporting the middle in his left hand. While striking the side of the bin he felt a pop in his right shoulder.

THE EMPLOYEE POSITIONED THE HOPPER ON A FORKLIFT ABOVE THE DUMPSTER. HE PUSHED UP ON THE RELEASE WITH HIS RT. HAND. HE TRIED TO ASSIST THE DUMPING ACTION WITH HIS LEFT HAND ON THE HOPPER. THE HOPPER STARTED FORWARD THEN ROLLED BACK MASHING HIS FINGER.

EE WAS WIPING OUT A GLASS BREAKER (SMALL) WITH A PAPER TOWELL USING HIS RIGHT INDEX FINGER AND MIDDLE FINGERS. THE BREAKER BROKE CUTTING THE TOPOF HIS INDEX FINGER BETWEEN THE FIRST JOINT AND KNUCKLE.
