Unloading roof bolt supplies out of scoop bucket onto #2 roof bolter when the scoop bucket pressure leaked off slowly onto the top of the left foot causing pain to developed in his left foot. The injured refused medical treatment and refused to be transported to medical facility for treatment.

Picked up 2 bundle of roof plates and put on bolter. Started bolting put up 3 rows back started hurting, burning in lower back.

After excessive rainfall and flooding the # 3 belt entry of the mine was blocked & canopy was crushed when materials fell from the highwall. Also, fan was hit but no damage to it except alarm & chart system.
