EMPLOYEE WAS AIR ARCING A BALLAST COMPARTMENT INORDER TO CUT A HOLE IN IT. THE LID BROKE LOOSE SOONER THAN EXPECTED AND FELL ON THE EMPLOYEE'S RIGHT LEG.

EE WAS WORKING ON THE SCREEN AND DIDN'T BOPTHER TO CLIMB OFF THE EQUIPMENT CORRECTLY. HE WANTED TO SPEAK TO ONE OF THE OTHER MENT THAT WAS STANDING JUST BELOW THE SCREEN. SO HE JUMPED. AS HE W AS COMING DOWN HE CAUGHT HIS LEFT HEEL ON A PIECE OF I-BEAM. CARELESSNESS WAS THE CAUSE OF THIS INJURY.
