HE TOLD ME AT END OF WORKDAY HE DIDN'T FEEL GOOD. ON HIS WAY HOME HE GOT SICK AND AFT HE GOT HOME HE WENT TO THE HOSPITAL. THE DR. TOLD HIM HE HAD A HEAT STROKE. HE ADMITTED HIM TO THE HOSPITA L.
HE WAS WALKING DOWN A BANK TO GET ON THE CRANE. HE SLIPPED AND FELL HE PULLED A LIGAMENT IN HIS RIGHT KNEE.

HE WAS HELPING LIFT A COUNTERWEIGHT OFF A SHAFT ON A SHAKER SCREEN COUNTERWEIGHT WAS SET DOWN ON HIS RT RING FINGER HE HAD TO HAVE HIS FINGER NAIL REMOVED & TWO STICHES TAKEN IN HIS FINGER

REPLACING BEARLING SANT PLANT HE WAS TRYING TO GET THE BEARING TO GO INTO PLACE. HE GOT HIS HAND BETWEEN THE BROWING AND THE SCALPING FRAME HE CUT HIS RING FINGER AND HIS LITTLE FINGER ON HIS LEFT HAND HE HAD TO HAVE STICHEN IN HIS FINGER.
