I.NAME WAS SETTING JACKS ON WILCOX MINER - AND SLIP OF ROOF FELL ON HIM.
THE ROOF FALL OCCURRED MAINLY BECAUSE THE PREVIOUS OPERATOR DID NOT DETECT THE ADVERSE ROOF CONDITIONS WHICH WAS A SLIPPERY HORSE BACK THAT COULD OF BEEN DETECTED IT PROPER PRECONDITIONS HAND BEEN TAKEN DURING THE BOLTING CYCLE THIS FAL DID NOT IMPAIR VENTALATION OR IMPEDE PASSAGE THE FALL MEASURED 35 FT LONG 20 FT WIDE AND 3 1/2 TO 4 FT HIGH
