EE was replacing shanks on a loader bucket. EE got a bolt partially loose and it dropped out of square shank. EE held it up with hand and when EE hit it with the impact, it spun and cut EE's right hand.

Employee was cutting a piece of torn belting that was wrapped around a head pulley shaft, when the utility knife slipped. He cut the inside of his left wrist.

Recordable hearing loss.

EE was leaning over the undercarriage attempting to grab his lifting chain while installing the belly pans. EE was leaning directly over the lifting eye. He grabbed the his chain and was backing out away from the undercarriage when the lifting eye caused a contusion to his left chest area.

Working on a cone crusher changing liners. During a procedure of setting nuts into a tramp iron system, worker got his right hand 1st digit finger pinched setting one of the nuts.

EE WAS LIFTING BLOCKING AND SCREEN & FELT SOME DISCOMFORT IN HIS BACK & LEGS. HE FINISHED THE DAY WORKING. NEXT DAY, HE HAD A LOT OF PAIN IN HIS LOWER BACK.

EMPLOYEE WAS WALKING XX AWAY FROM HIS TRUCK WHEN HE SLIPPED AND FELL ON HIS LEFT SIDE INJURING HIS SHOULDER.

EMPLOYEE WAS LIFTING OBJECT WHILE HIS RT FOOT WAS ON UNEVEN GROUND AS HE LIFTED HIS RT ANKLE TURNED OVER SPRAINING THE ANKLE

EE WAS SWINGING A MALL TRYING TO GET A ROCK REMOVED FROM JAW IN CRUSHING PLANT. HE FELT SOMETHING POP IN HIS BACK.

EMPLOYEE WAS HOLDING A BAR WHILE ANOTHER EMPLOYEE WAS USING A SLEDGE HAMMER TO HIT THE BAR TO LOOSEN A ROCK STUCK IN A TAIL PULLEY THE HAMMER BOUNCED OFF THE BAR BACK UP INTO THE EMPLOYEE HOLD ING THE BAR MOUTH PRELIMINARY DAMAGE ARE TWO CHIPPED TOP FRONT TEETH

WHILE CLOSING THE LITTLE JAW, EMPLOYEE WAS PUSHING ON WRENCH WITH FOOT TO TIGHTEN WHEN EMPLOYEE TWISTED, PINCHING NERVE IN BACK.

EMPLOYEE WAS TRYING TO UNPLUG THE CONE ON THE CRUSHING PLANT WITH A SPECIAL ROD. WHEN HE PUT THE ROD INTO THE CONE, THE ROD FLEW DIRECTLY UP INTO HIS FACE.

AFTER LOADING A PIECE OF EQUIPMENT ONTO THE TRUCK TRAILER, HE WAS LOADING SOME TIMBER USED TO LOAD THE PIECE OF EQUIPMENT. WHILE PLACING THE TIMBER DOWN ON THE TRAILER, HIS LEFT LITTLE FINGER WAS PINCHED CAUSING THE TIP OF THE FINGER TO BE BROKEN AND CAUSING A CUT DOWN THE SAME FINGER.
