Blondy Mining Inc · Struck against a moving object

THE SC HIT THE RIB CAUSING THE SC TO BOUNCE, CAUSING OPERATOR TO JAM HIS HEAD INTO THE CANOPY CAUSING INJURY TO NECK AND BACK OF THE OPERATOR. THE OPERATOR HAD INJURED HIS NECK AT ANOTHER OPER ATION WHICH REQUIRED SURGERY IN MAY OF 1996. THEINJURY OCCURRED 9/24. HE CAME BACK TO WORK 9/25 ON 9/27 HE WENT TO DR. AND IS UNDER PHYSICIANS CARE.
