EE WAS OPERATING CRUSHER WHEN A ROCK (18 X 18 X 18) CAUGHT ON THE CONVEYOR AGAINST THE CHUTE. EE WAS STANDING ON ROCK PILE BESIDE CONVEYOR AND HAD DISLODGED ROCK BY HAND. EE WAS BALANCING ROCK ON SIDE RAIL WHEN HIS FOOT SLIPPED. THE ROCK DROPPED, PINCHING HIS FINGERS AGAINST A BRACKET.
EE WAS HOLDING ONTO A CABLE HE WAS USING TO LOWER A REPAIRED TOGGLE PLATE IN TO POSITION ON THE JAW CRUSHER. THE PLATE CAUGHT AND WHEN EE GOT IT TO MOVE IT DROPPED QUICKLY AND PULLED HIS HAND, PINCHING IT BETWEEN THE CHAIN AND SOLID METAL.

EE WAS CHANGING BATTERY ON DJB AT QUARRY. CABLE TERMINAL WAS TIPPED AND CORRODED. EE STRUCK IT WITH HAMMER TO LOOSEN AND FALKES WERE AIRBORNE AND STRUCK HIS EYE. DEBRIS FLUSHED OUT ABOUT 6-7 H OURS LATER BUT CAUSED SOME INFLAMMATION.
EE WAS PERFORMING CLEAN UP AND SOME MAINTERNACE WORK. HE WAS STANDING ON A 12X12" BLOCK OF WOOD TO REACH A CLEAN OUT AREA AND THE BLOCK TIPPED. HE LANDED ON HIS SIDE ON SOME 4" ROCK.
EE WAS LIFTING UP TAILGATE ON DUMP TRUCK IN ORDER TO FREE UP & LATCH GATE PROPERLY WHEN HE DROPPED THE GATE, BUT DIDN'T GET HAND OUT OF THE WAY & PINCHED FINGERS BETWEEN TAIL GATE & BOX. HE SH OULD HAVE PULLED ON THE GATE WITH THE ATTACHED CHAINS.

EE WAS MOVING ROCK & MUD FROM RAMP APPROACH & SURROUNDING AREA WHEN BUCKET TOOTH UNCOVERED A FRAYED PIECE OF WIRE CABLE THAT HAD BEEN BURIED. PIECE 1'DIA X 24" L, HE COULD NOT DISLODGE IT FROM TOOTH BY MANIPULATING BUCKET SO GOT DOWN,UNHOOKED IT, & THREW IT TO A DEBRIS AREA, WHEN THE WITH CUT HIS FINGER.
AFTER MAKING REPARIS ON THE BOOM OF HT E980C LOADER BUCKET, THE EE STEPPED BACK TO THE PLATFORM NEXT TO THE CAB AND BUMPED HIS HEAD ON THE MINER MOUNTING BRACKET.
EMPLOYEE WAS WORKING OUTDOORS ON A STORMY DAY. HE WAS PULLING WITH A COME-A-LONG TO RELPLACE DRIVE BELT OF FEEDER MOTOR. HE WAS STANDING ON THE TRAILER SUSPENSION & HAD ON LUG SOLE BOOTS. WHIL E PULLING/EXERTING LEVERAGE, HIS FEET SLIPPED & HE FELL 3 1/2 - 4 FT. TO THE WET GRAVEL, LANDING ON HIS LOW BACK.

EE SHUT OFF JAW & MOVED OVER BY BRIDGED AREA TO BREAK IT LOOSE B Y STRIKING & PRYING WITH A PICK. SEVERAL ROCKS CAME LOOSE & AS HE CONTINUED TO WORK, ONE THAT HE HAD NOT IDENTIFIED, DROPPED DOWN & LANDED ON HIS FOOT.
SHAKER SCREEN WAS RAISED USING KOMATSU EXCAVATOR. SCREEN WAS BLOCKED WITH 6 X 6'S, JACKS & SHIMS [PLASTIC TREE FALLING WEDGES]. AFTER NEW SPRINGS WERE INSTALLED, WHEN PRESSURE WAS RELEASED TO REMOVE JACK, SHIM POPPED OUT & HIT EMPLOYEE ON BRIDGE OF NOSE.
