Walking on wet scale walkway. Left leg slipped and twisted right leg/knee. Outboard side of scale walk is 80'x3' with 4' handrail. First 33' was covered with roll roofing for anti-slip. EE exited cab and was walking on non-covered surface.

EE's were loading shot under suprvision of blaster. They wre running an air compressor to blow water from previous days rain out of holes drilled in bench. Previous rain on lip above & vibration from compressor caused several rock to fall striking EE on leg.

Employee had gotten into pickup bed to remove dewatering hoses to prepare for a shoot. The morning was cold/frosty and he was taking extra caution as he climbed out using the bumper as a step. Both feet were on the bumper, with one hand on tailgate. His right foot slipped, causing him to fall to the ground with right knee bent to side.

EE was reaching for a piece of scrap metal that was in a trailer made from a pick up box when he heard/felt a pop on left rib. note: due to previous auto injury, ee has had 2 cervical vertabrae fused, which makes for limited motion when reaching.

EE WAS PUSHING SHOT ROCK TO LIP OF BENCH AFTER DIGGING LOOSE ROCK AWAY FROM BACK OF BANK. OCCASIONALLY, SMALL LOOSE STONES FELL. WHILE BACKING, EE HEARD A "PING" AND LOOKED AROUND JUST AS A ROCK FLEW AT HIM FROM THE BLADE, STRIKING HIM IN THE MOUTH AND CHIN. THE ROCK MAY HAVE BEEN THROWN FORWARD BY THE TRACK AND RICOCHETTED OFF THE BLADE.

CRUSHER WAS SHUTDOWN FOR MAINTENANCE. EE DOES NOT RECALL ANYTHING HAPPENING. BUT LATER HIS FOOT WAS SORE AND WHEN HE TOOK OFF HIS SHOE THE TOE WAS SEVERLY BRUISED & SWOLLEN. 3 WKS LATER, AFT ER BRUISE HAD DISAPPEARED & SWELLING DOWN, THE TOE NO LONGER FLEXES OR EXTENDS. THEN EE REMEMBERED THAT THE ORIGINAL INJURY WAS OFF THE JOB ANDOFF SITE. HE STUBBED HIS TOE ON CONCRETE.

WENT DOWN INTO JAW AREA AND LOADER OPERA TOR DUMPED LOAD OF SANDSTONE BOULDERS ON VICTIM.LOADER OPERATOR SAW VICTIM'S ARM WAVING AND GOT UP TO HELP CLEAR DEBRIS.

HE WAS OPERATING A JACK ON DOWN SIDE OF TRACK TO GET IT INTO PLACE, WHEN CHAIN DETACHED FROM TRACK, KNOCKING OFF HARD HAD AND STRIKING SCALP.

EE WAS CHANGING TIRE THUMB ATTACHMENT ON JD 790 EXC. HE WAS HAMMERING THE PIN THROUGH THAT WOULDSECURE IT IN PLACE. THE TWO PARTS WERE NOT COMPLETELY ALIGNED AND THE IMPACT WITH THE BACK END O F THE AX HEAD RESULTED IN STRESS TO THE MUSCLES OF THE ABDOMEN.

EITHER THE ROCK SHIFTED OR HIS ANKLE ROLLED AND HE FELL FACE DORWARD INFORWARD INTO A DEPRESSION ABOUT 4' DEEP, STRIKING SHIN AGAINST ROCK.

3 EE WERE RE-ATTACKING TRUCK BOX TO CHASSIS. BOX HAD BEEN LOWERED BY CHAIN WITH BUCKET OF 235, WHICH WAS HOLDING IT STEADY. INJURED EE WAS HANDLING ON GROUND TO SIDE OF DRIVERS CAB, WITH A PRY BAR. THE BOX MAY HAVE HUNG UP AND EE CAUSED IT TO SHIFT BY PUSHING OR MOVING PRY BAR. EE S FINGER INJURED WHEN DRIVER STEP ON BOX BUMPED INTO CAB TRAPPING SEVERING THUMB.
