EMPLOYEE WAS IN THE PROCESS OF INSTALLING A ROOF BOLT. WHEN HE PUT HIS LEFT HAND ON THE TIGHTENING WRENCH, THE WRENCH SLIPPED CAUSING HIS LEFT INDEX FINGER TO BE CAUGHT BETWEEN THE WRENCH AND THE ROOF BOLT.

THE EMPLOYEE WAS POUNDING A ROOF BOLT DRILL STEEL AGAINST THE ROOF BOLT MACHINE HE WAS OPERATINGIN ORDER TO DISLODGE DUST THAT HAD PLUGGED THE STEEL. HE STRUCK HIS LEFT INDEX FINGER AGAINST TH E MACHINE, RESULTING IN A BROKEN BONE.
