EE was climbing up on the haul truck at the quarry. Rock was being loaded into the bed of the truck. When EE stepped-up, EE felt a pop/slip in the back of the left knee. When EE tried to put weight on EE's leg, EE had severe pain.

EMPLOYEE WAS DRIVING LOADER UP TO #2 ROCK PILE WITH BUCKET TOO HIGH. EE STEPPED ON GAS AND BRAKE PEDAL AT THE SAME TIME. LOST CONTROL AND LAID THE LOADER ON ITS SIDE. NO INJURY- BROKEN WINDOW ON LOADER.

EE was walking from Genst to conveyor #17, slipped on a patch of ice, fell backwards onto back and right side by trying to catch himself. EE hurt right lower back, right side and elbow.

Moving water pump suction hoses EE slipped on rocks and fell to the ground catching self with his arms. EE injured his right shoulder. EE just thought it was a jam at the time. EE did not go to a doctor until now since he was still having problems with it.

Getting out of loader, missed step and fell to ground onto right shoulder.
Picked up rock that was too heavy and felt strain around top of shoulder area or the top of arm (right arm) reported accident to other supervisor on 2-21-05. did not want to see dr at that time. on 3-7-05, ee called manager, and requested to see dr because the shoulder was still bothering him. appointment 3-8-05 2:00 pm.

WAS GETTING OFF LOADER, STEPPED ONTO CHOCK BLOCK, CAUSING HIM TO TWIST & FALL AGAINST THE CONCRETE. LANDED ON RT. SHOULDER AND SKINNED UP KNEES. 12/30/04: EE WAS STILL HAVING PROBLEMS WITH RIGHT SHOULDER. DIAGNOSIS: TORN ROTATOR CUFF. HAD SURGERY 12/17/04. EE WILL BE OFF WORK SEVERAL WEEKS.
UNSURE, EMPLOYEE GOT DIZZY AND WENT HOME. HE APPEARED TO BE FLUSHED FROM HEAT EXPOSURE.
A non mine truck driver was getting out of his 5 ton dump truck to check the tarp when his foot missed the step, he then fell & landed on his back. He was taken to the Katy Clinic & they found no apparent injury. Pain persisted & x-rays were taken again on 6-29-04, these x-rays showed 4 cracked ribs.
