The employee was assisting another employee reconnect a hoist jack to boom on a 988 Cat loader. The employee was holding the jack in line, while the other employee was lifting the jack with a service crane. The chain slipped allowing the crane rope to swing. The winch block struck the employee in the head resulting in a laceration. Treated at E.R.

The employee was operating a bulldozer, cleaning up the quarry floor. He was backing up when the rear of the dozer slid over a 5' bank, upsetting the dozer - it came to rest on its left side. The employee suffered a fracture to the third (ring) finger on his left hand. The physician treated the fracture with a splint. The employee was released to normal duty.

Ee was using a hand crank to tighten a screen. His left thumb became entangled in the crank handle & was twisted. He did not seek medical attention until 12/5/05. The dr diagnosed sprain & returned him to full duty. On 3/22/06 he saw an Orthopedic specialist who determined he would have to have minor surgery on 3/29/06. No lost days, no restricted days.

Employee was performing maintenance-greasing: he was carrying a grease gun. He stopped to kick the mud off of his boots and dropped the grease gun on his left foot. He was wearing sted toe boots, but the gun hit directly behind the protection. Inident resulted in contusion to little toe, little foot.

EMPLOYEE WAS SCRAPING OUT CRUSHER, HE HAD USED A LADDER TO CLIMB INTO CRUSHER. AS HE ATTEMPTED TO CLIMB OUT, HIS FOOT SLIPPED ON LADDER CAUSING HIM TO FALL. HE GRABBED THE UPPER EDGE OF THE OPENING, WHICH KEPT HIM FROM FALLING. HOWEVER, AS HE WAS HANGING HIS BODY SWUNG BACK INTO THE LOWER EDGE OF THE CRUSHER, STRIKING THE EDGE. EE OVEREXTENDED HIS BACK.

EE was assisting mine foreman in repairing cracks in feeder- hopper (by welding). Both ee's were inside the hopper welding. As a result, the reflection of the radiant light from the welding burned the ee's eyes.
