SUPERVISOR WAS ATTEMPTING TO ACCESS SCRAP METAL, USING A CONSTRUCTION FORKLIFT TO REMOVE ROOFINGMETAL SHEATS FROM ON TOP OF THE SCRAP METAL. UPON PULLING ON THE SCRAP METAL THE ROOFING METAL FELL OFF OF THE FORKS OF THE LIFT STRIKING THE SUPERVISOR JUST ABOVE HIS LEFT ANKLE.
TWO EE'S WERE ASSIGNED TO REMOVE A PLATE/DOOR FROM A MACHINE AFTER REMOVING THE BOLTS ONE EE ON THE INSIDE STRUCK THE DOOR TO KNOCK IT LOOSE. THE PLATE FLEW OFF STRIKING THE INJURED TRAPPING HIS LEFT HAND BETWEEN THE PLATE AND THE MACHINEHE PULLED HIS HAND AS IT WAS BEING TRAPPED
INJURED WAS CUTTING ONE PIECE OF STEEL OFF OF ANOTHER PIECE OF STEEL BEAM. ON COMPLETING THE CUT, THE SMALLER PIECE FELL FREE ALLOWING THE BIGGER PIECE TO FALL SOME 2" STRIKING THE LEFT FOOT O F THE INDIVIDUAL. THE BEAM STRUCK THE FOOT JUST BEHIND THE STEEL TOE CAP OF HIS FOOT. THE INJURED REMOVED THE STEEL FROM HIS FOOT AND SOUGHT ASSISTANCE.

THE INJURED PERSON WAS USING A PRY BAR TO HELP SEAT A STEEL LINER. THE PRY BAR WAS NOT SUFFICIENTLY LINED AT EDGE OF THE METAL, RESULTING IN IT SLIPPING OUT FROM THE BAR AND WENT DOWN HIS HAND AND STRUCK A METAL KNIFE BLADE, MASHED LITTLE FINGER. FINGER WAS BROKEN. FINGER DID NOT REQUIRE SETTING THE BONE DID NOT REQUIRE A SPLINT.
AIR ARCING-SLAGE LANDED ON NECK CAUSING JERKING REACTION, ARM GOT CAUGHT ON SHARP STEEL BLADE OF PUGMIXER. TAKEN TO HOSPITAL, ER DR. APPLIED SUTURES TO CLOSE WOUND.
