U.S. Silica Energy Company · Fall from machine

It is believed that the customer truck driver fell from EE's trailer to the ground while stepping from the trailer to the safe rack. The fall resulted in multiple injuries requiring medical attention and a loss of work days.

Employee fell while preparing to fuel a D6 dozer after a ladder/stool failed, resulting in a right ankle fracture.

While descending a stairway, the EE missed a step and landed hard on the outside of right foot, resulting in a fracture to the metatarsal bone. The fracture wasn't discovered until a clinic visit on 7/14/25.

Injured had just dismounted their haul truck with both feet on the ground. When the employee turned to walk away the left knee buckled. Diagnosed with a left knee strain.

While attempting to clean the windshield of a haul truck in contradiction of company policy the employee fell to the ground. The fall resulted in a fractured rib and facial bones.

The employee was dumping a dust collector hopper. The hopper would not return to the locked position, so the employee tried locking it manually. In doing so, EE's finger was caught between forklift mast and hopper, transported to the hospital: tuft fracture and stitches to EE's thumb.

Due to employees' failure to follow company policy for fall protection, EE fell off a truck. The employee was transported to the medical facility, where they received stitches to right knee. EE also has lacerations on chin, shin, a bone chip in wrist, and a chipped tooth.

The employee was using a pipe wrench to manually lower the thickener tank rake when the mechanism broke free EE's finger was smashed between structure and wrench.

Two EEs were changing out the screens at Rotex 5186. In the process of removing a screen the injured EE received burns on forearms. 1st AID measures were taken by plant personnel, recommended by Axiom Medical Service. Subsequently, EE was seen by MD for follow up evaluation on 9/18/19. The MD would not allow the EE return to work on EE's next assigned work day of 9/19/19

EE claims injury occurred on 8/5/19. EE reported to Supervisor until 8/25/19. EE was taken for medical eval on 8/26/19, released to full duty. On 9/10/19 EE complained pain returning and was transported for second medical eval and was released to full duty. MRI was scheduled & was obtained on 10/10/19. 11/5/19 EE returned for further medical evaluation, assigned restrictions.

Employee was removing the bucket pins from excavator to place the bucket on another excavator. After the pin was driven out completely the stick moved down and struck their right hand

Miner became weak, dizzy and nauseated. Co-Worker assisted miner until local EMS was notified and responded then transported EE to local emergency room. Miner was released 1 1/2 hours after arrival with heat exhaustion as diagnosis.

EE placed right hand on lid of dust bin as forklift was setting down. EE sustained minor injury. EE was offered medical treatment and declined. On 4/27/19 EE requested medical treatment and was seen by MD. Treating MD could not see issue and released to full duty. On 5/2/19, MD determined diagnosis was incorrect and a fracture was diagnosed.

Electrician was using a utility knife to strip insulation from a wire to make a repair. Employee removed cut-resistant gloves prior to beginning the task. Employee cut right thumb with the utility knife, requiring seven sutures. No lost time and no restrictions.

Employee finished installing the last fastener during installation of a wire mesh conveyor guard. EE took off gloves to install the fastener. Employee stood up from kneeling beside the guard panel, EE's bare hand brushed across a spliced area of the expanded wire mesh on the guard panel. EE's right finger received two extensive lacerations which required stitches.

Employee was in a 126' boom lift attempting to relocate the boom lift unaware the basket was lodged on structure. When basket of boom lift broke free the basket jerked causing employee who was holding on to handrails of basket to smash right hand between basket rails and structure causing employee to smash/pinch right hand and 5th fingertip.

Employee reported employee felt something on knee 11/21. EE told co worker it might be a boil. Two days later EE sought treatment. 11/26 EE reported to work with a release and complained of worsening symptoms. EE left job site and went to the hospital where EE was admitted for cellulitis.

While attempting to connect an SRL to the harness, the employee lost balance and fell (same level) striking left knee on a toe board. The resulting injury was a laceration requiring 5 stitches.
