THE INJURED EMPLOYEE WAS ATTEMPTING TO FUEL THE MAIN WATER PUMP. HE STEPPED UP ON THE LEFT REAR TIRE OF THE TRAILER THAT THE PUMP IS MOUNTED ON, TO REACH THE FUEL TANK. THE TIRE WAS WET FROM D EW, HE SLIPPED AND FELL OFF OF THE TIRE, LANDING ON HIS BACK ONTO A MOUND OF SAND OVER A PIPE IN THE GROUND.
THE INJURED EMPLOYEE WAS BACKING UP THE FUEL TRUCK TO THE RIP RAP MACHINE TO REFUEL IT, AFTER FUELING IT, HE WALKED BACK TOWARDS THE FRONT OF THE TRUCK & LOST HIS FOOTING ON SOME LOOSE GRAVEL & FELL; LANDING ON HIS RIGHT KNEE.
INJURED EE WAS HOLDING CONVEYOR BELT IN PLACE UNDER CRUSHER ON UNEVEN GROUND WITH LOOSE DIRT. HE WAS IN A SEATED POSITION AND HIS WEIGHT SHIFTED ALLOWING HIM TO SLIDE A FEW INCHES STRAINING HI S BACK.
LOWER CURTAIN WEAR BLOCK SLIPPED FROM ONE EMPLOYEE'S GRIP AND IT FELL AND HIT ANOTHER EMPLOYEE'S FOOT.
MINER WAS DISMOUNTING LOADER WHEN HE LOST BALANCE AND JUMPED FROM SECOND STEP UP THE LADDER.
MINER WAS WELDING ON CRUSHER. HIS FOOT SLIPPED AND HE STRAINED HIS BACK.
HE STEPPED OFF BOTTOM STEP OF LADDER TO PORTABLE CRUSHER ONTO UNEVEN GROUND AND TWISTED HIS KNEE WHILE CHECKING OIL LEVEL ON PORTABLE CRUSHER.
MINER WAS DRIVING A PICKUP NEAR A SAND PILE WHEN FRONT TIRE CONTACTED EDGE OF SAND PILE. MINER DID NOT HAVE FIRM GRASP ON STEERING WHEEL & IT TURNED, JAMMING HIS FINGER ON STEERING WHEEL.

OPERATOR WAS ON THE BACK OF THE 950F LOADER SERVICING ANOTHER PIECE OF EQUIPMENT. THE LOADER ROLLED AND PINCHED EMPLOYEE BETWEEN HIS LOADER AND THE EQUIPMENT HE WAS SERVICING.
OPERATOR WAS CLIMBING THE LADDER OF HIS CAT 966 LOADER. ON HIS ASCENT HE SLIPPED OFF THE THIRD STEP & HIT HIS KNEE ON THE SECOND STEP.
