Employee stated that while coming out of a kneeling position they felt their knee "pop". Employee stated that there was no signs of swelling and that there was no pain. Employee finished out the remainder of the shift.

EE was driving haul truck when EE began to get a headache accompanied by fatigue and dizziness. EE was taken to the hospital and was diagnosed with mild heat exhaustion. EE was cleared of any other issue and was released without restrictions.

Employee reported they fell from the steps of the sweeper on or about 4/6/2022. It was later discovered that the miner sought medical treatment on their own with a chiropractor without notifying mine management of the need for medical treatment.

Employee performing welding operations inside a temporary structure on-site at customer facility. Employee was wearing a 3M auto-shade welding helmet set at shade level 10 while performing welding operations. After end of shift; later in the evening employee felt discomfort in eyes. Employee sought treatment, and was treated for welding flash.

Employee was putting a piece of channel iron in the back of a truck. As ee lifted it into the truck a skid steer sounded back up alarm which startled ee. As ee checked to see where the skid steer was ee dropped the channel on right thumb which was resting on the bed of the truck. This resulted in a laceration that required stitches and a fracture to the tip of thumb.

EE stated that while dismounting the final step on the Komatsu 785 EE misjudged the distance which resulted in a fall at the same level. On 5/8/17 EE reported that intermittent pain in the left shoulder was to the point that EE needed to seek medical treatment. The employee was given work restrictions that the operation could not accommodate, thus resulting in a lost time injury.

Employee was on an 8-foot step ladder replacing v-belts for the VSI, when stepping off ladder onto the ground, employee states he twisted his right knee and ankle stepping on loose 3" rock. The employee sought medical attention on 9/4/15 diagnosed with a knee/ankle strain and given restrictions.

A maintenance employee was assigned to perform a front axle repair on a CAT236B skid-steer. While removing the front left tire the EE got his left hand 5th finger pinched between the wheel stud and the steel rim of the tire. The EE was diagnosed with a laceration and two fractures of the distal phalanx. He was released and returned to work with restrictions.

EE was replacing impact rollers on C3 & loaded 2 rollers into manlift (1 on the floor, and 1 lying unsecured on the top rail). EE hit uneven spot in road causing basket to jerk & roller to slip off the rail striking EE's rt foot. EE had X-ray & diagnosed as a contusion. EE returned on 12/15 w/out restriction. On 12/16 diagnosis was changed to a fracture of 5th proximal phalange.

EE took break to walk/stretch legs. The EE then climbed the access ladder onto the haul truck and felt a pull in his lower back from pulling himself up onto the bottom step (33" high). The EE tried to care for his back himself however the pain was not going away, EE went to clinic on 6/4/08. He was diagnosed with a back strain, given physical therapy and modified work restrictions.

Employee opened inspection doors on the VSI crusher to inspect wear on the hammers, manufacturer does not have means of securing open door. The opened door fell when accidentally struck by a come-a-long chain falling on the employee's left foot resulting in a fracture.

Employee was installing steering cylinder on a Komatsu WA-600. Employee had placed ram end in back half of machine and was in the process of pushing up on front end to align it in position. While pushing on cylinder employee felt sharp pain in left shoulder. Original diagnosis on 12/5/05 was shoulder strain, then on 1/10/06 after MRI diagnosed as rotator cuff tear.

EMPLOYEE WAS CARRYING 20-25 LB STEEL PLATE BETWEEN TWO CONVEYORS. UPON SETTING DOWN THE PLATE, THE EMPLOYEE DISMOUNTED THE SECOND CONVEYOR BY JUMPING TO THE GROUND. AFTER LANDING ON THE GROUND , THE EMPLOYEE INFORMED HIS FOREMAN THAT HIS BACK WAS CAUSING HIM PAIN. EMPLOYEE WAS SENT TO HOSPITAL WHERE HE WAS DIAGNOSED WITH A BACK SPRAIN AND RELEASED TO RETURN TO WORK WITH LIMITATIONS

EE WAS DESCENDING LADDER OF MICHIGAN 270 LOADER WHEN HE SLIPPED. HE ATTEMPTED TO JUMP BACKWARDS TO CONTROL THE FALL & LANDED ON HIS FEET. MOST OF THE FORCE OF THE FALL WAS ABSORBED BY HIS LEFT FOOT CAUSING A STRAIN TO THE FIRST TOE. HE WAS PRESCRIBED CRUTCHES AND PUT ON NO-WORK STATUS.

TRUCK DRIVER LOST AIR PRESSURE IN BRAKE SYSTEM NEAR THE TOP OF THE RAMP AFTER DUMPING AT THE PRIMARY CRUSHER. DRIVER ATTEMPTED TO STOP THE TRUCK BY SHIFTING TO REVERSE. THIS STALLED THE ENGINE AND THE DRIVER LOST CONTROL. THE TRUCK ROLLED ABOUT 580", THROUGH A BERM, OVER A 32" WALL, LANDED ON ITS NOSE AND FELL TO THE RIGHT SIDE. DRIVER WAS WEARING A SEATBELT.
