Employee was resetting #18 tail. He had a chain hoist anchored to an exposed roof bolt. We the employee applied pressure to the hoist, it caused the bolt to break striking him in the back of his head. This caused a laceration around 1 1/2" which required 7 staples.

Unplanned roof fall occurred on #17 conveyor belt line at #47 crosscut and adjacent neutral entry. Fall in belt entry approx. 100' X 20' wide X 6-8' high. In neutral airway was approx. 400' X 20' X 6-8' high. No injuries or entrapment or equipment involved. No ventilation involved. Escape way and travel way were blocked.

Employee was operating roof bolter, he has just gotten into operators deck and turn machine on, he had to adjust his body position in deck and had hold of canopy. The roof bolt machine move and he caught his three fingers between top of canopy and the mine roof.

Unplanned roof fall occurred that are approximately 20 ft. wide x 6 ft. High x (from spad 758 to 764). No injuries or entrapment involved, no equipment involved. Blocked Walkway to #2, #3 and #4 DPP. No escape way involved still have flow of air in return. The exact location is 2nd southeast panel at spad 758.

Employee was installing the outside roof bolt which had a curtain hook on it. The employee stated that the bolt was 3 feet from top and he was trying to position curtain hook while rotating the bolt. The curtain hook caught his finger and glove resulting in tearing his end of his right index finger.

An unplanned roof fall has occurred in the #6 entry on 2nd panel at between spad #1200 and spad #1210. Fall was approx 160' long X 20' wide X 6' to 7'high. Area was bolted with 6' resign bolts. There were no injuries involved and the fall did not affect ventilation or any travel ways.

Employee was walking across section (seam height approximately 44") he was bent over at the waist. A roof bolt had been damaged and he was in the process of pointing it out to Supervisor & another employee when his hand struck the plate, resulting in a laceration. He received 2 sutures and returned to work at full duty the same day 4/26/11.

Employee was outby section getting a load of cement blocks to construct a stopping. He turned his head (seam height 53 inches and employee 6 ft 2 inches) and struck a fly pad board that was sticking down slightly. Material from board and roof fell behind safety glasses into his eye. He was given prescription eye drops and released to return to work at full duty.

Member was swinging drill boom in when he started to fall, he was trying to catch himself by grabbing the top of the canopy with his left hand. Member continued to fall into the control levers and raised canopy into roof causing lacerations to his middle finger, which lead to doctors amputating the tip of his middle finger.

After further investigation it has been determined that a reportable accident occurred to this employee. He was loading a flinger type rock duster with dust on the end of a supply car. When raising the rock dust bags up (40 lbs.) he felt a pulling sensation in upper abdomen. He missed work from June 23, 2010 until July 11, 2010.

EE was bolting top when the drill steel became lodged. He dropped the head and struck drill steel to free it, but was unsuccessful. Raised drill pot and engaged rotation to free steel. Drill steel was not properly seated and when is came out of hole struck his left wrist.
