EE was doing product change over at the three deck screen. EE was standing on the belts below. The upper half of chute was open and not secured properly. EE grabbed the chain used to open it and pulled the upper half over onto where EE was standing.

The bottom rung of the access ladder broke off when the operator stepped on it while exiting the machine, thus falling 3-1/2' to the ground.

Miner was grinding links on a feeder chain and got a metal shaving in EE's eye. EE said that it did not start irritating EE until 11/12/19 when EE went to the ER after work.
Scale Operator was stung by a bee. Scale Operator is allergic to bees. Scale Operator was taken to the Hospital for observation.

Worker was replacing grizzly bars and worker lost balance and smashed finger when grizzly bar dropped.
Miner was tightening bolts using a hand wrench and impact gun. Wrench slipped off a bolt hitting miner in the ribs. Miner did not go to the doctor; completing EE's shift. Miner worked a full shift on Saturday, 8/6/16 then went to the doctor after EE's shift. No lost time. Miner was not restricted from any duties.

Miner slipped in mud and rolled ankle. Supervisor was informed but Miner indicated that he felt fine. However, by 11-4-15, Miner's ankle had swollen to the point that Miner sought medical attention.
Employee reached into screen to remove buildup (rock / dirt) and Lacerated arm.

Miner was cleaning out chute of a screen; cut his arm while pulling arm out of chute.
Miner lifted a chain up a ladder, straining his lower abdominal muscles on Wednesday, 12-3-14 but did not report an injury. On 12-5-14 miner came to the office reporting soreness and then went to the doctor at 8:00 am the same day.
Sale Operator claims that on 11-13-14 she slipped on ice on the scale deck t-bar hurting her left heel. She did not report the accident and did not seek medical attention until 1-16-15 when she went to urgent care after work. She reported to work the next work day; she did not lose time / work day due to injury.

Employee was holding a bar over his right shoulder while his Supervisor was hitting the bar with the hammer. The Supervisor didn't hit the bar square, which caused the hammer to kick to the left, and hitting the employee on the bottom part of his nose.

Standing on a step stool changing fuel filters on a haul truck when the stool gave way and he fell. He made contact with the upper part of his body and the ball joint on the haul truck.
Just walking around on uneven ground & crawling up on equipment irritated an old back injury.

While climbing down off of a D8K, employee slipped on a push beam, caught himself and twisted his left wrist.
Employee was cleaning from under the screen trailer with a shovel. He stepped down on a rock and bent his ankle to the left. It didn't start hurting until the next day.

EMPLOYEE REPORTED BROKEN SEAT WAS THE CAUSE OF HIS INJURY TO HIS BACK.
