Miner was pulling down safety guard on bob cat and caught EE's finger.

Miner was driving haul truck on 4/7/15 and claimed the haul road was bumpy causing strain to lower back and hamstrings. He did not go to the doctor but reported to work on 4/8/15 (the next day); working as the Feeder Operator as a protective measure. On 4/9/15 Miner went to his Chiropractor who placed him on Light Duty.

Supervisor called for a ambulance because of employee's condition. Employee was transported to McMinnville Hospital. Company is waiting to hear what the problem his.

EE had been working for a few hours when he went to his Supervisor and stated that he was feeling lite headed and he thought he had Vertigo again. He stated that he had someone coming to pick him up and take him home. We were informed later in the evening that the Doctors thought that he might have had a mild stroke and they were admitting him to the Hospital.
Climbing down a ladder and twisted his knee.

Employee was adjusting the chutes on the screen plant, pulling out of chutes; the slide came out and fell on his finger smashing it between the slide and pry bar.
Employee was installing V-belts in the scalper screen, using a digging bar for tension. The bar gave way and he fell to the ground injuring his knee.

WE WERE MOVING PALLETS OF POWDER FROM TRAILER TO TRUCK. I WAS RUNNING THE PALLET WHILE 4 OTHERS WERE PUSHING. AS WE ENTERED THE TRUCK THE TRAILER TIPPED OVER. ONE PERSON STRUCK THE TOP & SIDE;
AS A GROUP WE WERE MOVING PALLETS OF POWDER FROM TRAILER TO TRUCK. AS WE MOVED A PALLET I WAS RUNNING TH EPALLET OTHER EE AND ANOTHER WERE PUSHING ALONG WITH OTHER. AS WE ENTERED THE TRUCK THE TRAILER. STARTED TO TIP AND THE WHOLE TRAILER TIPED OVER EE STRUCK THE TOP AND SIDE. OTHER WENT OUT THE BACK AND HIT THE TRAILER OR THE GROUND.
TRYING TO LAY A BOARD IN FRONT OF FEEDER TIRE.
RUNNING UP THE STAIRS TO THE SCALE SHACK, DID NOT USE HANDRAILS AND FELL ON SHOULDER.
