THE AREA AROUND THE WATER TRUCK WAS MUDDY DUE TO HEAVY RAINS AND THE FILLING OF THE TRUCK. AFTER THE TRUCK WAS FULL, THE DRIVER WAS LIMBING INTO THE CAB WHEN HIS FOOT SLIPPED FROM THE STEP. HE CAME DOWN AND HIT HIS CHEST ON THE FUEL TANK.
AFTER UNLOADING HIS FLY ASH VEHICLE NO 1 WAS PARKED TO CLEAN OUT HIS TRAILER THE DRIVER WAS IN THE TRAILER (STANDING ON THE GROUND BECAUSE OF IT BEING A BOTTOM DUMP TRAILER) VEHICLE NO 2 WAS B ACKING UP COMING AT VEH NO 1 AT A 45DEG ANGLE NOT SEEING VEH NO 1 UNTIL IT WAS TOO LATE VEH NO 1 WAS MOVED APPROX 1FT STRIKING DRIVER ON THE LEG
