Miner was adjusting the distance of the forks of the fork-lift attachment on the Kubota SV95 Skid Steer Loader, the fork slid contacting the Miners right thumb cutting it. Miner then took self to the hospital for care.

Was shoveling waste and turned and hit right hand on side of conveyor frame

Employee was digging out troughs with Volvo EC350 excavator. EE reached too deep with the bucket and pulled the excavator into a unlevel position.

Employee was using a pipe wrench to loosen bolt to remove valve at plant. Employee was stood on cat walk, valve was at chest level. Wrench slipped and employee lost balance and hit back on handrail causing a fracture to the right wing of the lower vertebra.

Employee reported injury as work related on 11-16-2018. Employee was out 6-13, 6-14, 6-15 and 6-18. Employee did not say time missed was worked related. Employee worked day of alleged injury. "I was helping another employee load the grease drum on the fuel truck and they lost grip and I had to catch it from falling back off" Back/Left leg pain.

Employee was picking up a piece of metal next to screener. Received a puncture wound without foreign body to the right hand.

hydraulics failed on Volvo 360 trackhoe, causing bucket to come to the ground, jarring the cab. Caused muscle pull in employees back.

Employee reported on 3/25/2010 that he thought he had something in his eye. Epithelial abrasion to right eye.

Pulled left calf muscle when climbing up onto trackhoe.

Mechanic was working on Mack road truck adjusting clutch. He asked the other mechanic to depress clutch from inside the cab. The mechanic had not moved his hand when he asked the other mechanic to depress clutch and when the clutch was depressed the mechanic caught his middle finger on his right hand severing the top of his finger .

Employee was climbing into dump truck, his foot slipped on bottom step and he fell onto his back. Low back pain, hip pain and muscle strain.

Employee fell from loader while climbing out. Conditions damp/raining. Employee does not remember how she fell. Landed in the sand, refused to lay still, got to her feet, refused back board when attended to by medical personal. Transported to the ER for back pain. Diagnosis compression fracture of L2. Employee rec'd training on 5-11-07 practices for getting on and off equipment.

Employee was tightening bolts on screen at plant when his tool slipped and his arm came back and he hit his left elbow on plant. Contusion to left elbow area.

Employee was sitting in buddy seat of the Case off road haul truck. He reached across his body with his right hand to close the door which was on his left. He shut the door on his left arm. Contusion to left elbow area.

EMPLOYEE WAS TURNING A BOLT WHEN PUTTING IN A PIN IN THE BUCKET OF THE VOLVO EC360 EXCAVATOR. THE WRENCH SLIPPED AND HE HIT HIMSELF IN THE FACE WITH THE WRENCH CAUSING A LACERATION TO THE LEFT SIDE OF HIS FACE.

Employee was picking up a 50-lb bag of chemical when he strained his back. He has a prior history of back problems that we were not aware of until this injury occurred.

Employee was greasing equipment at beginning of shift. He threw the grease hose over the bucket of the front end loader. He caught his left fore arm on the edge of the bucket during the motion of throwing the hose. He received a laceration of the left fore arm upon contact.

EE WAS OPERATING A FRONT END LOADER AND SAID HE STRUCK A HOLE, SAID HE HIT HIS HEAD ON ROOF OF LOADER AND JARRED HIS BACK, SAID HE WAS WEARING SEAT BELT, SITE WAS INSPECTED BY SUPERVISOR. NO H OLE FOUND. DOCTOR IS STATING THAT THIS IS A QUESTIONABLE CLAIM AT THIS TIME.

WAS USING A HAMMER (HAND HELD) TO KNOCK IN A PIN AND HIT HIS HAND INSTEAD OF THE PIN.

WAS SCRAPING HAUL ROAD WITH TRACTOR & BLADE. DROVE TRACTOR OFF SIDE OF ROAD. TURNED TRACTOR ONTO ITS SIDE.

DRIVER CAME OUT OF BREAKROOM. A 2ND EMPLOYEE WAS BACKING OUT OF PARKING SPOT. NEITHER SAW THE OTHER. BUMPER OF PICKUP HIT DRIVER.

EMPLOYEE FAILED TO ENSURE THAT BED WAS DOWN ON TRUCK AND DROVE THROUGH POWERLINE.

WORKING ON EXCAVATOR, FINGER PINCHED ON MOVING PART.

SLIPPED ON CATWALK AT PLANT.

WAS LOOKING UNDER STACKER BELT TO SEE IF THERE WAS ANY SAND BUILD UP AND BUMPED HIS HEAD.

BACKING FRONT END LOADER AND ACCIDENTLY HIT A OFF ROAD DUMP TRUCK (PARKED)

PULLED LOWER BACK PICKING UP ROLLER AT PLANT.

EE WAS GETTING OUT OF BACKHOE AND HE SLIPPED AND FELL.

EE WAS SHOVELING UNDER A GUARDED PULLEY. HE HIT THE UNDERSIDE OF THE PULLEY WITH THE SHOVEL AND THE SHOVEL SPRANG BACK FRACTURING HIS FINGER. EE WAS TERMINATED FOR FAILURE TO PRODUCE URINE SPE CIMEN.

EE SLIPPED ON SOME SLIPPERY STEPS EXITING THE PORTS TRAILER AT THE BIG PLANT. HE HAD A PREEXISTING BACK AILMENT PRIOR TO THE FALL THAT IMPAIREDHIS ABILITY TO WALK CONFIDENTLY THE FALL RESULTE D IN WORK LOSS & RESTRICTED DUTY. THE INJURIES AQUIRED FROM THE FALL WERE BACK SPASMS & CHRONICBACK PAIN

WAS CLIMBING OUT OF THE LOADER AND MISSED A STEP, FELL TO THE GROUND AND HIT HEAD.

EMPLOYEE WAS GETTING DOWN OFF OF DOZER AND HIS FOOT SLIPPED OFF THE STEP AND TO GROUND CAUSING INJURY TO KNEE.

WAS OPERATING DOZER & THE MOTION CAUSED A MUSCLEPULL IN THE GROIN

GOT GREASE IN EYE WHILE GREASING EQUIPMENT DID NOT REPORT UNTIL 10/6/99

WAS CLEANING OUT TRUCK, CAUGHT THUMB BETWEEN SEAT & SEAT LEVER.

SNAD BLEW INTO EMPLOYEE'S LEFT EYE. CAUSED IRRITATION. EMPLOYEE WENT TO DOCTORS'S 12-4-97. PARTICLE OF SAND WAS REMOVED. HE WAS REFERED TO AN EYE CLINIC TO CHECK THAT THE EYE IS OK.

EE WAS FUELING BACKHOE WHEN HE GOT SOMETHING IN HIS EYE. HE DID NOT THINK ANYTHING WAS WRONG BUT LATER IN THE EVENING IT REALLY STARTED HURTING SO HE WENT TO THE EMERGENCY ROOM. THEY REMOVED A SMALL OBJECT FROM HIS EYE AND REFERED HIM TO A EYE CLINC.

EMPLOYEE WAS DRIVING BOTTOM DUMP WHEN TIE ROD END CAM EOFF BALL AND STEARING AND CAUSED TRUCK TO RUN INTO BANK. PRESSURE OF SEAT BELT CAUSED BRUISING OF ABDOMINAL WALL. EMPLOYEE DID NOT REPO RT DISCOMFORT UNTIL 7-16-97.

EE WAS PUTTING A POWDERBASED CHEMICAL INTO A TANK AND THE WIND BLEW IT INTO HIS RIGHT EYE.

EMP WAS REMOVING LOG FROM THE TRAILER OF THE OFFROAD DUMP TRUCK. THOUGHT HE COULD PUSH LOG UP AND OVER CENTER BRACE BUT IT SLIPPED AND FELL BACKONTO HIS ARM AND HAND.

EMPLOYEE WAS WORKING AT THE PLANT CHANGING A SCREEN WHEN HE GOT DIRT IN HIS EYE.

EE WAS CLIMBING INTO LOADER USING STEPS AND HANDRAIL WHEN HE SLIPPED AND LOST HIS FOOTING.

EE WAS WORKING AROUND PLANT; & GOT INTO POISION OUS OAK & THEN HE RUBBED HIS EYES WITH HIS HANDS &THE POISION SPREAD

EMPLOYEE WAS MOVING 1 5LB SAND BAGS &SHEINCURRED A LOWER LUMBER STRAIN

LIFTING A WATER PUMP.

WAS OUT SPLITTING HOSE AND KNIFE SLIPPED.

EMPLOYEE WAS WORKIN GON BELT SHE DID NOT APPLY SAFETY DISCONNECT LOCK ON CONVEYOR MTOOR. ANTOEHR EMPLOYEE TURNE DON CONVEYOR BELT.

INNJURED WAS PULLING REAR END (HOG HEAD) OUT OF MACK TRUCK USING PRY BAR. PRY BAR SLIPPED OUT FROM UNDER REAR END CAUSING BLOW TO HEAD RESULTING IN HEADACHE.

LOADER OPERATOR HIT A POT HOLE & HURT HIS BACK

DRIVING A BUSHING INTO PC200 TRACKHOE, HE MISSED BUSHING & HIT HIS KNEE.

PERSON WAS CARRYING SCREEN CLOTH. HE TRIPPED & FELL & DROPPED SCREEN ON HISSELF.
