Miners were removing a counterweight from an excavator, standing side-by-side, when the counter-weight dropped unexpectedly to the ground.The victim stumbled backwards,fell landed on the back of his head on the shop floor. He was unresponsive during CPR. The autopsy report concludes the proximate cause of death as"commotio cerebri"noting also cardiac arrest.

Miner was filling bulk diesel tank on service truck, miner completed task and stepped down from back of truck. Miner rolled left ankle, suffered a sprained ankle.

Miner was adjusting the distance of the forks of the fork-lift attachment on the Kubota SV95 Skid Steer Loader, the fork slid contacting the Miners right thumb cutting it. Miner then took self to the hospital for care.

Employee was digging out troughs with Volvo EC350 excavator. EE reached too deep with the bucket and pulled the excavator into a unlevel position.
Employee was using a pipe wrench to loosen bolt to remove valve at plant. Employee was stood on cat walk, valve was at chest level. Wrench slipped and employee lost balance and hit back on handrail causing a fracture to the right wing of the lower vertebra.

"I was helping another employee load the grease drum on the fuel truck and they lost grip and I had to catch it from falling back off" Back/Left leg pain.

Mechanic was working on Mack road truck adjusting clutch. He asked the other mechanic to depress clutch from inside the cab. The mechanic had not moved his hand when he asked the other mechanic to depress clutch and when the clutch was depressed the mechanic caught his middle finger on his right hand severing the top of his finger .

Landed in the sand, refused to lay still, got to her feet, refused back board when attended to by medical personal. Transported to the ER for back pain. Diagnosis compression fracture of L2. Employee rec'd training on 5-11-07 practices for getting on and off equipment.

Employee was sitting in buddy seat of the Case off road haul truck. He reached across his body with his right hand to close the door which was on his left. He shut the door on his left arm. Contusion to left elbow area.
EMPLOYEE WAS TURNING A BOLT WHEN PUTTING IN A PIN IN THE BUCKET OF THE VOLVO EC360 EXCAVATOR. THE WRENCH SLIPPED AND HE HIT HIMSELF IN THE FACE WITH THE WRENCH CAUSING A LACERATION TO THE LEFT SIDE OF HIS FACE.

Employee was picking up a 50-lb bag of chemical when he strained his back. He has a prior history of back problems that we were not aware of until this injury occurred.
Employee was greasing equipment at beginning of shift. He threw the grease hose over the bucket of the front end loader. He caught his left fore arm on the edge of the bucket during the motion of throwing the hose. He received a laceration of the left fore arm upon contact.

EE SLIPPED ON SOME SLIPPERY STEPS EXITING THE PORTS TRAILER AT THE BIG PLANT. HE HAD A PREEXISTING BACK AILMENT PRIOR TO THE FALL THAT IMPAIREDHIS ABILITY TO WALK CONFIDENTLY THE FALL RESULTE D IN WORK LOSS & RESTRICTED DUTY. THE INJURIES AQUIRED FROM THE FALL WERE BACK SPASMS & CHRONICBACK PAIN
WAS CLIMBING OUT OF THE LOADER AND MISSED A STEP, FELL TO THE GROUND AND HIT HEAD.

EE WAS FUELING BACKHOE WHEN HE GOT SOMETHING IN HIS EYE. HE DID NOT THINK ANYTHING WAS WRONG BUT LATER IN THE EVENING IT REALLY STARTED HURTING SO HE WENT TO THE EMERGENCY ROOM. THEY REMOVED A SMALL OBJECT FROM HIS EYE AND REFERED HIM TO A EYE CLINC.
EMPLOYEE WAS DRIVING BOTTOM DUMP WHEN TIE ROD END CAM EOFF BALL AND STEARING AND CAUSED TRUCK TO RUN INTO BANK. PRESSURE OF SEAT BELT CAUSED BRUISING OF ABDOMINAL WALL. EMPLOYEE DID NOT REPO RT DISCOMFORT UNTIL 7-16-97.
