WHILE OFF-LOADING SCREEN DECKS, EMPLOYEE WAS ATTEMPTING TO ATTACH A CHAIN TO A SET OF 3-DECKS WHEN HE BUMPED THE TOP DECK CAUSING IT TO SHIFT UNEXPECTEDLY. EMPLOYEE WAS BRIEFLY CAUGHT BETWEEN TWO DECKS BEFORE HE FELL TO THE GROUND. HE WAS TREATED FOR MULTIPLE LACERATIONS AND CONTUSIONS. PHYSICIAN IMPOSED RESTRICTIONS PREVENTED RETURN TO WORK.
EE'S LEG WAS CAUGHT BY THE UNANTICIPATED MOVEMENT OF A CONVEYOR BELT WHILE HE WAS BREAKING A ROCK THAT WAS WEDGED IN THE TAKE-UP PULLEY. THIS RESULTED IN A RIGHT LEG FRACTURE.

EE NOTICED A SECOND EE FALLING FFOM A SHORT ELEVATION AND CAUGHT HIM. THEY BOTH FELL TO THE GROUND. MEDICAL ATTENTION WAS NOT SOUGHT UNTIL LATER FOR BACK STIFFNESS.
EE WAS REMOVING A COME-A-LONG FROM THE DRILL STEEL CAROSEL ON A ROTARY DRILL. THE STEEL & COME-A-LONG SHIFTED UNEXPECTEDLY & CAUGHT THE EE'S RIGHT RING FINGER BETWEEN THE COME-ALONG & THE CARO SEL.
