At approximately 9:20 am, two employee's were making a pipe connection on the dredge. Injured employee's left leg was pinched between the floating pipeline and the bow end of the work boat crane base.

Annual audiogram results that were analyzed on 5/28/2018 indicated a reportable hearing loss in the worker's left ear, averaging 25.67 dB at 2000, 3000 and 4000 hz. This has been an upward trend over the previous 3 year period and this year it crept over the 25 dB reportable level.
Improper load positioning on work boat crane causing the boat to list to one side resulting in boat capsizing.

Miner had received audiogram results indication a reportable hearing loss. On 7/17/2015, the miner was retested and the results were similar necessitating the need to report.
Dredge operator was helping move the dredge from one mining area to another. upon positioning the dredge, the operator spun a ball float so that he could reconnect the float to the shore line. When he did the float pinched is hand between the hull and the float cutting the middle finger of the left hand.

Miner was cutting a 2" piece of rubber hose with a retractable razor knife. Upon completion of the cut, the knife, held in his right hand, slipped and stabbed him in the back of his left hand. There was a puncture wound about 1/2" in length and considerable swelling under the skin.

Miner was changing a troughing idler on a radial stacker from an elevated platform. He removed the old idler, placing it in a truck from which he was working. Upon completion of the job, he climbed down, stepping on the center roller of the removed idler, turning or rolling his ankle

EE was welding a fitting in a tank from a water truck, there was a spotter outside in the tank. The EE took a break and left a propane torch hanging in the tank. When he returned, struck the lighter in the tank and there was an ignition. Post investigation revealed that EE had left the torch valve partially open and had not turned on the fan prior to reentering the tank.

EE has had trouble with right ear since baseline test in 2000 due to a recurring occlusion. Tests erratic. On 5/7/07 testing indcated significant hearing loss & the possible presence of a tumor. EE checked by MD, cleared & rechecked by audiologist on 7/31/07 with similar results. EE wishes recheck by MD. Enrolling him in HCP as a precautionary measure. Losses of 25.67 db in R ear.

DRIVER FOR A HIRED TRUCK WAS IN THE CAB OF HIS TRUCK WHILE BEING LOADED. THE LOADER WAS OVER THE TRUCK AND LOST HYDRAULIC PRESSURE CAUSING THE LOADED BUCKET TO CRASH DOWN ON TRUCK. THE DRIVER HIT HIS HEAD ON THE CABL GLASS AND WAS LATER DIAGNOSED AS HAVING SLIGHT CONCUSSION.
EMPLOYEE WAS BEING LOADED BY OUR LOADERMAN WHEN A PIECE OF FROZEN DIRT BROKE LOOSE FROM BUCKET AND WENT INTO BED OF HIS TRUCK CAUSING A JOLT TO TRUCK CAUSING A JERKING TO HIS BACK.
