EMPLOYEE HAS COMPLAINED OF NECK AND BACK PROBLEMS CAUSED BY THE BOUNCING OF THE LOADER HE HAD DRIVEN FOR YEARS. HE HAS BEEN GETTING CHIROPRACTIC CARE FOR A FEW WEEKS. HE CAME TO THE OFFICE ON NOV. 29 TO FILL OUT WORKER'S COMP. PAPERWORK.
