Employee was walking up entry and felt a pain and burning in left knee , EE advised pain increased with any movement , also advised there was no mechanism of injury. Employee was brought to surface and transported to medical facility for treatment

A unplanned roof fall occurred on the 2 North Mains at Break 47,in the #1 Entry , 120 Ft Inby Spad 3512. There was NO Injuries, did not affect ventilation or passage of persons. Fall is being dangered off and breaker timbers are being installed at all approaches. Fall Measures 80' Long , 18-20' wide , 6-10'high

An Unplanned roof fall occurred in the #5 Entry in the North Mains at Break 18-19, Fall is approximately 3,000 ft from section, Fall Measures 20'W X 50'L X 6-10' H. The fall will not be rehabilitated, fall did not affect ventilation or passage of persons. All entrances have been dangered off and cribbed.

Fall did not affect ventilation or passage of persons, fall will not be cleaned up, all entrances have been dangered off and cribbed.

Employee was checking terminal connections in the number one mine cathead. EE placed the Allen wrench into the bolt on the number three power lead and received an electrical shock from stored energy in the cable. EE was taken to the hospital, treated for numbness and pain in left arm. EE stayed twenty four hours for observation.

EE was pushing the drill steel to the roof. As EE let go of the drill steel when EE was starting the hole, the joy stick stuck in rotation and up pressure. The drill steel bent and came out of the hole spinning and struck EE in the right hand 3 times before EE could get away from it.

A roof fall was discovered when the foreman was doing his weekly airways. The fall occurred in panel No. 6 and ran from #4 ehtry through the crosscut into #5 entry outby spad #1975. The fall measured 80'L x 20'W x 6'H. There were no injuries or equipment damage. The area had been bolted with 42" resin bolts.

THE EE CONT'D TO WORK WITH NO MED TREATMENT UNTIL 1/3/05 & THEN STARTED MISSING WORK DAYS. THE INCIDENT IS UNDER INVESTIGATION TO DETERMINE IF AN INJ EXIST TO PREVENT EE FROM WORKING.

HE RAISED THE MANTRIP WITH A FORKLIFT & WHEN HE WAS LOOSENING THE SPRING BOLTS, THE MANTRIP TEETERED & THE END DROPPED CATCHING INJURED'S RIGHT HAND BETWEEN THE IMPACT WRENCH AND THE GROUND. WHEN HE PULLED HIS HAND FREE, HE LACERATED THE BASE OF THE MIDDLE FINGER. TRANSPORTED TO ER & RECEIVED 7 STITCHES.

Employee stated he was operating battery powered rubber tire mantrip and hit an offset with his right wheel. This caused the steering wheel to turn quickly hitting him on the right forearm, resulting in a fracture.

EE WAS ADVANCING CONTINUOUS BRIDGE SYSTEM AND WAS POSITIONED AT THE END OF THE TAILPIECE. THE CONTINUOUS MINER OPERATOR REVERSED THE CONTINUOUS MINER, FORCING THE EDGE OF THE DECK ON THE BRIDGE INTO THE TAILPIECE, WHICH RAISED IT INTO THE MINE ROOF. THE EE STRUCK THE MINE ROOF WITH HIS HEAD, CAUSING A STRAIN TO THE NECK.

NO INJURIES, ENTRAPMENT, EQUIPMENT OR VENTILATION INVOLVED. THE FALL WAS IN AREA THAT WAS BUTTED OFF. IT OCCURRED IN #3 ENTRY AT SPAD #433.

EE WAS HOOKING 3/4" BY 18' LONG CHAIN FROM CONT BRIDGE SYSTEM TO THE TAILPIECE. AS HE WAS PULLING AND TWISTING HIS BODY HE FELT PAIN IN HIS LOWER RIGHT SIDE. EE DID NOT MISS ANY DAYS OF WORK.
