A unplanned roof fall occurred on the 2 North Mains at Break 47,in the #1 Entry , 120 Ft Inby Spad 3512. There was NO Injuries, did not affect ventilation or passage of persons. Fall is being dangered off and breaker timbers are being installed at all approaches. Fall Measures 80' Long , 18-20' wide , 6-10'high

An Unplanned roof fall occurred in the #5 Entry in the North Mains at Break 18-19, Fall is approximately 3,000 ft from section, Fall Measures 20'W X 50'L X 6-10' H. The fall will not be rehabilitated, fall did not affect ventilation or passage of persons. All entrances have been dangered off and cribbed.

A unplanned roof fall occurred in the #3 entry in the #1 North Mains, between break #40 and #41 , 50' Outby spad #3476 fall measures 20'x 50' x 6-10'high.Fall did not affect passage of persons or ventilation. All entrances had been dangered off and cribbed prior to fall.

An unplanned roof fall occurred in the 1 North Mains outby the section 2800' at break number 23, 100' inby spad # 3359. Fall Measures approx 200' L X 20'W X 5-13' in height. Fall did not affect ventilation or passage of persons, fall will not be cleaned up, all entrances have been dangered off and cribbed.

Employee picked up a bucket of miner bits. When EE turned, felt a sharp pain in the lower back. Employee said it was an old injury from a previous employer.

Employee was walking through crosscut at break 36 on the #3 Belt line, slipped and fell landing on their left leg which resulted in an ankle fracture

Employee was installing Roof Bolts, while trying to insert drill steel in drill chuck EE felt something pull and a burning sensation in shoulder

Unplanned roof fall occurred on the # 3 Belt Panel, in the #1 Entry inby spad 3495 extending 2 breaks inby. Fall Measured 220'x20'x7-10'. Fall will not be cleaned up but area around fall will be rehabilitated

One break from the face the employee was trying to move a cable and felt pain in their back.

Employee was checking terminal connections in the number one mine cathead. EE placed the Allen wrench into the bolt on the number three power lead and received an electrical shock from stored energy in the cable. EE was taken to the hospital, treated for numbness and pain in left arm. EE stayed twenty four hours for observation.

EE was pushing the drill steel to the roof. As EE let go of the drill steel when EE was starting the hole, the joy stick stuck in rotation and up pressure. The drill steel bent and came out of the hole spinning and struck EE in the right hand 3 times before EE could get away from it.

Employee was standing between #2 entry and #3 Entry in crosscut. Miner was loading Shuttle Car in Crosscut 2 Left when a rock came out of conveyor of Miner striking EE in lower right leg.

Roof fall occurred on 2/7/2023 on #1 Section in the #3 entry face. It was approximately 12 feet in height consisted of mostly stack rock and mud streaks.

The Employee was trying to straighten a pie pan, began pulling it against their chest when they felt something pull in their right shoulder.

A roof fall was discovered when the foreman was doing his weekly airways. The fall occurred in panel No. 6 and ran from #4 ehtry through the crosscut into #5 entry outby spad #1975. The fall measured 80'L x 20'W x 6'H. There were no injuries or equipment damage. The area had been bolted with 42" resin bolts.

EMPLOYEE WAS HELPING PUT IN BELT STRUCTURE. A COWORKER HAD PUT ONE RAIL IN THE STAND. INJURED WAS TRYING TO PUT HIS IN. COWORKER THOUGHT INJURED HAD HIS IN AND HIT THE FIRST RAIL, THIS KNOCKED INJUREDS OUT, IT FELL AND SMASHED HIS FINGER

WEST VIRGINIA WORKERS' COMPENSATION COMMISSION AWARD GRANTED FOR 8.07% PERMANENT IMPAIRMENT RESULTING FROM NOISE-INDUCED HEARING LOSS.

EE WAS OPERATING BATTERY RUBBER TIRE MANTRIP. EE STRUCK COAL RIB CAUSING THE STEERING WHEEL TO SPIN SUDDENLY STRIKING THE EE ON THE LEFT HAND. THE RESULT WAS A FRACTURE TO THE RING AND MIDDLE FINGER ON THE LEFT HAND.

EE WAS PASSING BY SECTION SCOOP IN #4 ENTRY TOWARDS THE POWER CTR. THE RUBBER 2 MAN PERSONNEL CARRIER STRUCK THE OPERATORS COMPARTMENT OF SCOOP,CAUSING WHEEL TO SPIN STRIKING EE'S RIGHT HAND. THE EE CONT'D TO WORK WITH NO MED TREATMENT UNTIL 1/3/05 & THEN STARTED MISSING WORK DAYS. THE INCIDENT IS UNDER INVESTIGATION TO DETERMINE IF AN INJ EXIST TO PREVENT EE FROM WORKING.

EE WAS CRAWLING ACROSS CONTINUOUS HAULAGE SYSTEM (BRIDGE) AND STRUCK HIS KNEE. EE CONTINUED TO WORK AND DID NOT RECEIVE ANY MEDICAL TREATMENT UNTIL 10/21/04; AT THIS TIME THE OCCURRENCE BECAME REPORTABLE.

Caught fingers of right hand between boom and stab jack of roof bolter while attempting to clean dust hose and another operator moved roof bolter head.

INJURED WAS ATTEMPTING TO CANGE A BROKEN SPRING ON A RUBBER TIRED MANTRIP. HE RAISED THE MANTRIP WITH A FORKLIFT & WHEN HE WAS LOOSENING THE SPRING BOLTS, THE MANTRIP TEETERED & THE END DROPPED CATCHING INJURED'S RIGHT HAND BETWEEN THE IMPACT WRENCH AND THE GROUND. WHEN HE PULLED HIS HAND FREE, HE LACERATED THE BASE OF THE MIDDLE FINGER. TRANSPORTED TO ER & RECEIVED 7 STITCHES.

EE WAS INSTALLING ROOF BOLT AND PINCHED HIS FINGER BETWEEN BOLT AND ROOF BOLTER HEAD.

Roof fall occurred at no. 29 crosscut #4 entry (intake). Fall was approx 18' w by 10' l by 3 1/2' h. No entrapment, injuries or equipment involved. No escapeways or ventilation involved. Area was bolted with 42" fully grouted resin bolts.

Employee stated he was operating battery powered rubber tire mantrip and hit an offset with his right wheel. This caused the steering wheel to turn quickly hitting him on the right forearm, resulting in a fracture.

EE WAS ADVANCING CONTINUOUS BRIDGE SYSTEM AND WAS POSITIONED AT THE END OF THE TAILPIECE. THE CONTINUOUS MINER OPERATOR REVERSED THE CONTINUOUS MINER, FORCING THE EDGE OF THE DECK ON THE BRIDGE INTO THE TAILPIECE, WHICH RAISED IT INTO THE MINE ROOF. THE EE STRUCK THE MINE ROOF WITH HIS HEAD, CAUSING A STRAIN TO THE NECK.

ROOF FALL OCCURRED APPROXIMATELY 180' IN LENGTH, 22' WIDTH & 6'-7' IN HEIGHT. AREA WAS SUPPORTED WITH 42" RESIN BOLTS WITH 5' RESIN & 10' COMBINATIONS & CRIBS SPOTTED IN THE AREA. NO INJURIES, ENTRAPMENT, EQUIPMENT OR VENTILATION INVOLVED. THE FALL WAS IN AREA THAT WAS BUTTED OFF. IT OCCURRED IN #3 ENTRY AT SPAD #433.

EE WAS MOVING BRIDGE, HARD HAT HIT TOP, KNOCKING HAT OFF. EE REACHED TO GET HARD HAT SWINGING AROUND HITTING THE JOYSTICK & RAISING THE DECK. SMASHING HIS LEFT HAND BETWEEN DECK STOP & ROOF. HE SMASHED HIS HAND ABOVE THE WRIST & 4 FINGERS.INJURIES CONSISTED OF OPEN WOUNDS ON TOP & BOTTOM OF HAND & BREAKING 4 FINGERS.

BACKING UP MANTRIP AT HIT ELBOW ON LARGE GREEN MANTRIP. SMASHING AND BRUISE ARM.

EE WAS MAKING A HIGH VOLTAGE SPLICE ON CABLE. THE UTILITY KNIFE HE WAS USING SLIPPED AND CUT LEFT THUMB. MEDICAL ATTENTION ONLY, NO WORK DAYS LOST, LACERATION REQUIRED STITCHES.

EE WAS HOOKING 3/4" BY 18' LONG CHAIN FROM CONT BRIDGE SYSTEM TO THE TAILPIECE. AS HE WAS PULLING AND TWISTING HIS BODY HE FELT PAIN IN HIS LOWER RIGHT SIDE. EE DID NOT MISS ANY DAYS OF WORK.

EE WAS LANDING AN IDLER ROLLER ACROSS A TRUSS TO A SECOND EE FOR INSTALLATION AND FELT A SHARP PAIN IN HIS BACK. HE CLAIMS THAT HE HAD HAD REOCURRING PAIN SINCE THAT TIME AND DECIDED TO HAVE IT CHECKED OUT ON 9-12-03 AND THE SAFETY DEPT. BECAME AWARE OF THE DIAGNOSES ON 9-14-03
