HE WAS HELPING TO CHANGE A SCREEN ON THE SAND SCREEN. HE WAS HELPING PULL UP A PUNCH PLATE THAT WEIGHED APPROX 100 POUNDS AND TWISTED OR PULLED SOMETHING IN HIS LOWER LEFT BACK.

ROCK FELL OFF SCREENING PLANT & HIT HIM RIGHT LOWER BACK AS HE WAS BENDING OVER.
HE WAS TRYING TO LOOSEN TOP BOLT ON LOADER DRIVE SHAFT. THE SHAFT KEPT TURNING & HE TRIED TO HOLD IT STILL WITH ONE HAND WHILE LOOSENING BOLT WITH THE OTHER HAND. AS HE WAS DOING THIS HE FELT HIS BACK SLIP OUT OF PLACE. HE SAID HE HAS A PREEXISTING HISTORY OF A L-4 DISC. RUPTURE AND HERNIATED.
BENDING OVER MOVING CYLINDER IN ATTEMPT. TO GET IT ON BLOCKS SO IT COULD BE REPAIRED.
