At approximately 9:35AM employee was operating a tandem axel truck stockpiling material. Employee parked on a slope and began to dump. The box tilted causing the truck to fall onto its left side. The employee was able to exit the truck on their own. Employee experienced pain when walking so was taken to the ER for physician evaluation.

On 9/30/2015 EE received an additional restriction from a previous injury. His original restrictions were within his job duties but the new restriction of no climbing requires him be removed from his regular job making this a reportable injury. He previously received a first aid treated injury (right knee) when he lost 3pts of contact and fell from a scaler.

The fall was approx. 40'X40'X6' deep. There was slight damage to the elbow of the mine fan and to a electrical panel. This fall partially impedes airflow in the mine. The mine was immediately evacuated after discovery and MSHA was called.

At apporx 8:00am employee was moving boxed explosives from the magzine to the powder truck when he felt a pop & pain in his lower back. He continued working until 10:00am when he was carrying a box of explosives. He stepped on a rock while wading through water and felt intense pain in his lower back. He the reported his condition to Mgt. *Initial fax did not reach MSHA*

Emloyee has suffered increased hand cramping for ten years to the point he reported it to management on 11/20/2007. His condition, deemed carpal tunnel in both wrists, is not restrictive as yet, but will require surgery and a period of restricted duty recovery sometime in first quarter 2008.

EE WENT TO GREASE A HEAD PULLEY ON A CONVEWYOR. HE WAS CARRYING A GREASE GUN AND A TUBE OF GREASE. RATHER THAN CLIMB THE ENCLOSED LADDER PROVIDED TO CROSS THE HEAD PULLEY PLATFORM, THE EE WALK ED UP THE BELT. WHEN HE STEPPED FROM THE CONVEYOR FRAME TO THE PLATFOMR HE SLIPPED AND FELL INTO THE HANDRAIL BRUISING HIS LEFT SHOULDER.

MINE FOREMAN WENT DOWN THE RAMP TO THE MINE AND FOUND 6-7 TONS OF ROCK HAD FALLEN AT THE MAIN ENTRAMNCE TO THE MINE. A DRILLER WAS FARTHER BACK IN THE MINE AT THE TIME OF THE FALL. THE FALL WA S INSPECTED DEEMED SAFET TO CLEAR WITH A LADDER. THE DRILL WAS THEN USED TO SCALE DOWN LOOSE ROCK. THE BOOM TRUCK WAS THEN USED TO HAND SCALE.
