ROOF FALL OCCURRED, ONE CROSSCUT OUT BY SURVEY STATION NO. 810 ON NO. 1 ENTRY. THIS WAS A ROADWAY LEADING INTO A BLEEDER AREA. ROOF WAS FULLY BOLTED. FALL AREA APPROXIMATELY 50' X 20' AT IN TERSECTION.

THE ROOF FALL OCCURRED IN AN AREA THAT DIPPED INTHE MINE IN NO.5 ROOM OFF THE FIRST RIGHT BUTT ENTRY. ROOM WAS FULLY BOLTED ALSO EXTRA SUPPORT WAS SET UP IN THIS AREA. PRESSURE OVERRODE SUPPL EMENTARY SUPPORTS & CAVED.
MINER WAS LIFTING SHAFT OFF PERSONNEL CARRIER AND RUPTURED RIGHT SIDE. MINER WILL HAVE SURGERY AT A LATER DATE. THERE HAS BEEN NO LOST TIME TO DATE; WILL NOTIFY MSHA WHEN SURGERY OCCURS.

ACCIDENT OCCURRED IN #18 ROOM OFF OF THIRD RT ENTRY AND FLOODED THE CONTINOUS MINER DUE TO THE FACT MINE MAP WAS IN ERROR IN THAT LAST ROOM OFF OF SECOND RT ENTRY WAS DRIVEN 150 FT MORE THAN WAS SHOWN ON MAP
THIS ACCIDENT OCCURED 3-11-84 THE TIME IS NOT KNOWN BECAUSE NO PERSON WAS IN THE MINE WHEN THE ACCIDENT OCCURED THE CONTINUOUS MINER IS COVERED WITH ROCK. THE CAVE IS AT LEAST 20 FT HIGH WE FE EL WATER PRESSURE CAUSED THIS ROOM TO CAVE.
