Mechanic was adjusting idle on a skid steer (#L-59) Over reached his hand while attempting to make final adj/snug of adj screw, directly in-front of engine cooling fan. He was adj idle with engine running & the cooling fan struck his hand. He received a cut on right hand from the fan blade requiring 19 stiches to close. Engine should have been turned off during this task

While using a crowbar to replace idler arms on the pool sand stacker, the crowbar slipped and struck the EE behind the left ear.

Employee was preparing to get up on a loading machine. As he started to put his foot on the first step, he somehow missed the step which must have thrown him off balance. When his foot came down on the ground, the strain apparently caused a hernia in his groin area.

EE alleges that he tripped and fell while walking across the quarry. Nobody witnessed the alleged fall. EE has tried unsuccessfully (to date) to have a prior injury to the same shoulder covered under workers compensation.
EE was working in the quarry generator room and while attempting to pry a nail out of some wood, the pry bar slipped off the nail head causing some minor back strain as he apparently re-aggravated a pre-existing back problem.

EMPLOYEE WAS CUTTING DOWN A PIECE OF MORTAR WITH A CUTTING TORCH. A PIECE OF METAL FLEW INTO LEFT EYE.
BOUNCING AROUND IN A PAYLOADER CAUSED DAMAGE TO THE VERTEBRAE IN NECK. THE DAMAGE TO THE VERTEBRAE CAUSED A PINCHED NERVE.
WHILE DIGGING WITH THE PAYLOADER, HE HIT A ROCK CAUSING HIM TO SLAM FORWARD AND THEN BACK.
EMPLOYEE WAS CLEARING THE CONVEYOR BELT. RAN TO GET A SHOVEL, HIS HARD HAT HIT A STEEL BEAM, CAUSING HIS HARD HAT TO SLIP DOWN IN FRONT OF HIS FACE. THE BRIM OF THE HARD HAT CUT HIS LIP.
