Boxley Materials Company · Caught in, under or between running or meshing objects

Superintendent locked out and tagged out the crusher run belt to remove a blockage of material which had stopped the belt. The belt was cut on and off more than once during the process and the v-belts were replaced on the motor. The superintendent failed to disengage the starter switch during the last LOTO and their thumb was smashed in the v-belt.

Miner was helping to change the attachment on the excavator from a hammer to a bucket. As the miner tried to loosen the nut with a wrench, EE propped foot on the attachment for leverage. When the nut broke loose, the miner fell to the ground, landing on EE's elbow. The miner suffered a dislocated elbow.

LOADER OPERATOR BACKED AWAY FROM HAUL TRUCK TO GO FOR ANOTHER B UCKET W/O SEEING THE CONTRACTOR. WHEN HE DID SEE THE CONTRACTOR IN HIS MIRROR, HE SLAMMED ON HIS BRAKES TO AVOID HITTING THE CONTRACT TRUCK. HE STRAINED & PULLED HIS BACK MUSCLE FROM THE STOP
