THE SHUTTLE CAR WAS DUMPING ROCK ONTO FEEDER AND BIG ROCK GOT STUCK AT DISCHARGE END OF FEEDER. THE OPERATOR TURNED OFF THE MACHINE TO MOVE THE ROCK SO IT WOULD GO DOWN THE BELT AND HURT HIS BACK WHILE MOVING ROCK.

EMPLOYEE WAS CARRYING TWO 12 CM CAT PADS WHILE BENT OVER AND LOOKING DOWN. HE RAISED HIS HEAD RETURNING TO STANDING POSITION AND STRUCK HIS HEAD ON A ROOF BOLT THAT EXTENDED OUT APPROX. 2". EM PLOYEE SAID THAT HE FELT PAIN IN HIS LOWER BACK.
