Primary crusher was clogged. Employee was using a spud bar to dig out clog and hyper-extended their left elbow. Diagnosis was a pulled ligament.

Employee was in the process of changing a head pulley on the return conveyor belt. Despite having a sling around the shaft, when the mounting bolts were removed the head pulley shifted and employee's left hand was pinched between the pulley and framework. Employee left index finger suffered a laceration requiring 4 stitches.

Employee finished the shift but knee later swelled up and ee was unable to work on 6-13-17. Employee's PCP set up appointment with Orthopedic doctor on 6-20-17.
