Employee was removing a guard from a conveyor for maintenance, bolts were removed, guard was stuck, employee pulled hard, and lost balance and fell, hitting forehead on a rock on ground. Cut to Forehead as result.

EE failed to follow training and company required procedures. EE did not shut down, lock out/tag out equipment before removing a root from a moving converyor belt. Injury to top of left hand. Damage/Impairment unknown at this time.
