J & B Sand & Gravel LLC · Caught in, under or between (Not Elsewhere Classified)

This report is to provide add'l info for an incident that could've resulted in an EE's death. A worker was checking area & observed EE hanging from neck at sand classifier tank. EE was cut down & CPR started. 911 was called & White Co. First Responders arrived & took over. EE is still in St. Mary's Med Ctr. EE will be perm. disabled from the apparent suicide attempt. Notified MSHA.

J & B Sand & Gravel LLC · Fall from ladders

Employee was climbing fixed ladder to inspect bin level. Upon descending; hand slipped causing employee to fall backwards landing on concrete barrier block. Employee suffered fracture to L-1 vertebrae.
