INJURED WAS GREASING END LOADER WHEN HE BUMPED HIS HEAD ON LOADER BUCKET ARM. HE LATER DEVELOPED PAIN IN UPPER BACK & AFTER TAKING TREATMENTS FROM DR, HIS DR DETERMINED THAT INJURY WAS FROM PR EVIOUS ACCIDENT. AT THAT TIME, THE OFFICE WAS INFORMED.

LIME MILL DOOR WAS OPENED AND INJURED WAS INSPECTING HAMMERS FOR WEAR. WHEN HE TURNED THE ROTOR, THE HAMMERS COME OVER CENTER AND A 30# HAMMER HIT AND PINNED HIS ARM.
