Ee was welding on walkway above crusher on Fri, 6-11-04. At 1:00 pm ee alleges being hot, weak & not feeling well. Ee left work at 2:30 pm after not feeling any better. Ee not scheduled for Sat. or Sun. Ee reported to work on Mon 6-14-04. After 3 hours, ee complained of being weak & stomach pains. Ee left work to see his dr. Diagnosed as heat exhaustion & dehydration.

WHILE REMOVING GUARD FROM THE HAMMERMILL, HIS LEFT HAND SLIPPED AND THE GUARD FELL, STRIKING EE'S LEFT THIGH, LEAVING A CONTUSION.
GLOB OF GREASE ON CONCRETE PAD NOT CLEANED UP. EMPLOYEE APPARENTLY SLIPPED WHILE WALKING BACKWARD PULLING MOBILE 120# GREASE BARREL WITH AIR PUMP FALLING ON HIS BACK.

EE WAS USING A BOX CUTTER TO REMOVE INSULATION TAPE FROM ELECTRIC MOTOR CONNECTIONS THE BOX CUTTER SLIPPED THROUGH THE TAPE KNOT AND STRUCK THE EE S RIGHT THUMB AREA CAUSING A CUT. THE CUT REQ UIRED 6 STICHES TO CLOSE.

THE EMPLOYEE DESCENDED SCREEN 1 ONTO A SECTION OF GRATED CATWALK THAT IS 36" WIDE BY 14' LONG. HE HAD BOTH FEET ON THE CATWALK, WHICH IS LEVEL AND IN GOOD CONDITION. HE PICKED UP AN EMPTY PLAS TIC 2 GALLON JUG USED TO CARRY OIL IN AND TURNED TO WALK ACROSS THE CATWALK. THE EMPLOYEE STATED THAT AS HE TURNED TO WALK TO HIS RIGHT, HE FELT A PAIN IN HIS LEFT KNEE. HE STATED THAT HIS FO
EE WAS COMING OFF OF THE RAMP WHEN HE HIT A HOLE MEASURING 21FT WIDE BY 26FT ONG BY 5 INCH DEEP. THE AIR RIDE SEAT IN THE TRUCK FLEW UP, CAUSING HIM TO HIT THE TOP OF THE TRUCK CAB, RESULTING IN A NECK INJURY. SPRAIN.
INJURED WAS CLIMBING CATWALK LADDER TO ENTER THE TRUCK. HIS RIGHT FOOT SLIPPED ON THE SECOND STEP OF THE LADDER, CAUSING THE SUBJECT TO FALL BACKWARDS STRIKING HIS HEAD AND SHOULDER ON ANOTHER PARKED TRUCK AND THEN FALLING TO THE GROUND.

RAINY WEATHER WITH A LIGHT DRIZZLE FALLING. THE EMPLOYEE SLIPPED ON A STEP WHILE HE WAS HELPING TWO OTHER EMPLOYEES MOVE A STOVE USED FOR TESTING ROCK. THE EMPLOYEE SLIPPED ON THE 4TH STEP OF A SET OF 5 AND STRUCK HIS KNEE ON THE 2ND STEP. THIS RESULTED IN TORN LIGAMENTS IN HIS LEFT KNEE.
