He was standing on the conveyor belt platform, kneeling partly on the belt while loosening a belt clamp. The clamp released, slackening the belt, which caused him to lose his balance. There was sand on the belt platform which caused insecure footing and caused him to fall approximately 12 feet to the sand pile below, resulting in a shoulder contusion.
