Mellott Company · Struck by falling object

Employee was pushing the feeder back over the HP500 cone crusher. The plate that shuts the bin off had shifted enough to one side that it fell down, hitting employee on back lower part of leg below knee area.

Employee was using a utility knife to cut open the center strip of rubber on the 8 x 20 screen. Knife slipped and cut his left palm of hand.

Employee was cleaning up debris (dirt, dust) around the jaw crusher with a flat shovel for about 20 minutes. The next day he started complaining about back pain. The following day (March 23/2011)he came to work with the same problem and was sent to see a doctor.

Employee was running toward the plant. After taking several steps, he lost his footing and fell hard on his right arm. The right forearm began swelling immediately. The fall resulted in two (2) cracked ribs and deep bruising.

Employee was getting ready to pick up 200 lb shaft with crane when another contractor pulled on sling to tighten it. While employee was looking up at the hook being lowered, shaft rolled off stand and landed on right foot behind steel toe. Other contractor didn't speak up to alert employee.

Employee was walking toward electrical room at plant on upper level when he slipped on a patch of ice that was covered by a thin layer of snow. He landed on his left side. Was transported to ER and was found to have cracked two ribs and had deep brusing.
