TRUCK DRIVER WAS DRIVING AND STOPPED SHORT OF WHERE TO BE LOADD. TACKHOE OPERATOR TRIED TO GET HIM TO MOVE UP AND TRUCK DRIVER WASN'T PAYING ATTENTION SO TRACKHOE OPERATOR BUMPED TRUCK AND DR IVER SAID HE HURT HIS BACK.

PERSON WAS KNEELING ADJUSTING A SLURRY PUMP SUCTION SIDE LINER WHEN A HYDRALIC PUMP AND METER FELL OVER STIRKING PERSON MID TO LOWER BACK AREA. THE HYD PUMP METER ASSEMBLY WAS INSERTED INTO A 55 GALLONG DRUM. WHEN DRUM WAS EMPTY IT WAS A LITTLE TOP HEAVY AND FELL STRIKING PERSON.
