Miner replacing bearing on tail pulley. Miner struck bearing race with hammer and a piece of slag struck and cut left forearm.

Miner was utilizing a 1.5 ton chain come-along to straighten a piece of toeboard on a catwalk when the hook slipped off and struck him below the left eye. Resulted in two stitches below left eye.
