Miner was operating Cat haul trk when he ran off haul road striking a boulder berm that had been placed to protect trucks from a drainage ditch and culvert pipe. The truck crossed over a boulder hitting a dirt bank causing the operator to hit his head on cab frame

Miner was fueling and greasing D8N Dozer to start stripping burden. He was bending over to grease ripper on the back of the machine, as he straightened up, he pulled a muscle in his lower back.

Wet shot rock plugged up the Primary Crusher. I was inside of the crusher cleaning out the rock. I picked up a large rock to throw outside, the rock slipped off of the blow bar and fell on my left index finger, cutting it deeply.
