Employee was picking up a bundle of bolts off of scoop, one end of bundle fell off and landed on his right foot resulting in a fracture of a bone in his foot.
A roof fall occurred on MMU 002-0 last open crosscut in #4 heading. Fall measured 20' long, 20' wide and 4 to 5 feet thick.

EE WAS RIDING IN PERSONNEL CARRIER. GOING THROUGH A VENTILATION CURTAIN, EE'S KNEE PAD GOT CAUGHT ON THE CURTAIN. THIS CAUSED HIS FOOT TO GO UP INTO THE TOP (MINE ROOF).
DRIVING A PIECE OF KEY STEEL IN THE HEAD DRIVE MOTOR HE STRUCK HIS HAND WITH A HAMMER. STARTED MISSING WORK 6/23/04. COULD NOT GET INFORMATION FROM DOCTOR UNTIL 7/26/04 ON WORK INJURY THAT TOOK HIM OFF WORK.

HEPICKED UP A ROLL OF BELTLINE AND HE TWISTED AROUND TO THROW IT OFF OF A PERSONEL CARRIER. HE DID NOT START MISSING WORK UNTIL 1-29-04 FROM THIS ACCIDENT.
HE WAS HOLDING A PIECE OF DRIL STEEL AGAINST A CHAIN AND ANOTHER INDIVIDUAL WAS STRIKING THE PIECE OF DRILL STEEL WITH A HAMMER, WHEN A PIECE OF METAL FLEW OFF AND STRUCK HIM IN THE EYE.
STANDING ON TRACKS OF EXCAVATOR ATTEMPTING TO SHUT DOOR, DOOR CAME OPENED ON EXCAVATOR AND HE GOT OUT ON THE TRACKS TO SHUT THE DOOR, HE FELL OFF OF TRACKS TO THE GROUND.
