EMPLOYEE WAS HELPING MAKE A BELT SPLICE. HE REACHED TO MOVE THE ANVIL BEFORE THE OTHER MAN WAS THROUGH BRADDING THE SPLICE AND WAS STRUCK BY THE HAMMER. THIS RESULTED IN A LACERATED LT. INDEX FINGER. EMPLOYEE HAS BEEN RE-INSTRUCTED IN THE PROPER WAY TO MAKE A BELT SPLICE.

WHILE MAN WAS INSTALLING ROOF BOLT DRAW ROCK FELL FROM AREA BETWEEN LAST INSTALLED ROOF BOLT AND ATRS STRIKING MAN ON SHOULDER. ADDITIONAL SAFETY JACKS WILL BE USED IN AREAS PRONE TO DRAWROCK.
