EE FOUND THE FALL ON FIRE BOSS RUN AT 5:50 AM ON PRE-SHIFT EXAMINATION NO ONE WAS IN THE MINE. THE FALL MEASURED 35 FT LONG AND 25' WIDE IN INTERSECTION TO 18' IN X-CUT. IT FELL APPROX. 4' ON LIPS TO 6' IN THE MIDDLE.

IT WAS BOLTED ON 4' CENTE R 4' FULL GROUTED BOLTS. SLIP WAS IN IMMEDIATE ROOF AND WAS NOT VISABLE TO SEE THE FALL MEASURED 25' WIDE BY 24' LONG MAXIMUM HEIGHT 5'.
