Was attempting to remove a rock that was fouled in the #3 head. While using a pry bar to lift the rock, the bar slipped as I was pushing down causing my right hand to strike and get pinched by the bar on the edge of the #2 tail guard.

Employee was throwing rocks out of the feeder. He grabbed a hold of a big piece of sharp slate and threw it but before turning loose of it he caught his fingers between it and a solid piece of metal.

EE said he was looking back to see which scoop man was coming behind him. When a rock fell out the top and hit him in the back. He rolled out of the deck and grabbed the rock through it out of the deck against the rib got back in the miner and backed it out to take it to a new place. After a few minutes went by ee than said he was hurting and wanted to leave
Employee was installing fly boards in the #5 entry after drilling his starter hole. He was attempting to put his stack together when a piece of draw rock came out of the top smashing his left middle finger against the top opening at his pusher resulting in a severe laceration.
Had a double crack in 2 left that we were putting additional roof support up in the form of T3 straps (16 ft. long) when the boss left to go tend to the section for a moment. Employee attempted to install the T3 strap by himself, resulting in a strain of the back.

Cutting end off of belt cut finger.
Victim was riding on man trip with his foot vertical up and down. A low roof bolt was hanging from roof and caught his foot and rolled his foot up into the top between mine roof and man trip.
In #6 entry the rock duster was in the scoop bucket. Employee was pushing the rock duster over in the scoop bucket when the rock duster slid over it struck him in the knee having his knee caught between the rock duster and the ram.
Got in a Yellow Jacket Nest and was bee stung.
EE got finger fowled between roof bolt and roof bolt plate and cut finger.
Middle finger hit knob on steering wheel on small 4 wheeler ride (bottom end of finger had been cut off and had pins in finger years ago).
S/C dropped in hole and bounced up causing operator to hit head against top.
