Rooffall on #1 Beltline top was heavily laminated in that area. Area was cribbed off to prevent any access. Production was stopped at this mine. Equipment is being pulled to the outside.

Mining in area of laminated top and stack rock. When brk was cut through area of unsupported top fell pulling two rows of bolts in the #3 entry. Test holes in all out-by and face areas did not show signs of fall area. Area was cribbed and dangered off to prevent any more mining. There were no injuries.

Had a roof fall in #7 entry at brk 14 spad #73. Top is heavily laminated in area of fall. Additional cribs were built in affected area and surrounding areas. Area was completely dangered off. There were no injuries to report.

Had a continuation of previous fall in #4 entry brk #7 spad #28 whick took out intake stopping. Area was supported with jacks and cribs and stopping was rebuilt. Est fall 5'H-10'W-16'L.

Had fall at brk 14 spad #77 in #2 return entry. Fall was in area of laminated top. Additional cribs were installed and complete area was dangered off. Fall was approx 17' W x 20' L x 8' H. Fall was discovered during weekly run.

Had a continuation of fall in #3 entry that occurred 11/2/09. There were no injuries. Roof is heavily laminated in this area. Additional cribs were built in surrounding areas. Fall was inspected by MSHA and by State Offices.

Roof Fall in #7 entry approx. 2000' outby section. Fall was approx. 60'long X 18' wide X 6' high.Top was heavily laminated in arch of fall. Area was cribbed and dangered off from all directions. Stoppings were constructed around fall area.

Fall was discovered during preshift exam. Fall was in secondary escape way at brk 21 spad 109. Fall was in area of laminated top which was laced with additional support and cribs. All areas were cribbed and dangered off and secondary escape way was moved to another entry.

Had a continuation of roof fall in #2 return airways that occurred 9/9/09. Area had already been cribbed and dangered off. Additional cribs and dangered off boards were installed to keep people out of affected area. Fall was discovered while making return airway.

Had a portion of roof fall in #3 entry while mining. Roof contained surface cracks which were not visible due to mixed laminated top which was stuck to sandstone roof. Rock was approx. 3' to 4' in width, 3' to 4' in thickness and approx. 27' in length.

Had roof fall in #2 rm, 3 brks outby spad #320 in return airway. Top had several surface cracks and water was coming from cracks. Top had lamination of sandstone and slate.

Mare Development LLC · Caught in, under or between a moving and a stationary object

Operator dropped drill wrench in cradle of bolter. Operator kept raising and lowering pot until wrench was free - this action caused damage to machine - when he removed his wrench the boom on machine dropped hitting right leg.

Kettle bottom and some draw rock fell in the intersection of #2 entry and hit EE in the back of his calf, causing a laceration on his right leg.
