MINER WAS CHANGING PLACES AND THE TOP WAS REAL LOW. A PIECE OF ROCK HAD DROPPED, HANGING FROM A ROOF BOLT. EMPLOYEE DID NOT SEE IT. HE BACKED INTO IT, PUSHING HIS HEAD FORWARD AND DOWN, CAUSIN G HIM TO STRAIN HIS NECK. IT BECAME STIFF/SORE.
THIS MAN SAID HE HAD BENT A ROOF BOLT TO INSTALLIT AND WHEN HE WENT TO STRAIGHTEN IT UP HE PULLED SOME TENDONS IN LEFT ARM
ROCK FALL ON OFF SHIFT. NO ONE INVOLVED IN ACCIDENT.

BOSS WAS TRYING TO CATCH A PIECE OF DRAW ROCK WITH A BOLT AND MASHED HIS LEFT THUMB WIHT THE PLATE WHEN HE WENT TO TORQUE THE BOLT.
PIN MAN HAD PUT UP 1ST BELT IN CUT AND HAD GOT READY AND SWUNG PINNER FOR 2 MON BOLT AND A PIECE OF ROCK FELL OUT OF TOP AND HIT PIN MAN BELOW SHOULDERS AND DOWN HIS BACK WHEN HELPER SET ATRS FOR 2ND BOLT.
