Injured was backing scoop around the corner to reposition scoop, when the corner rolled & a piece of rock fell in the deck of the scoop striking his right arm at the elbow resulting in a bruise.
A roof fall was discovered at approximately 8:40 AM between spad 75637 and 75663 in the number 1 entry (left return). Dimensions of the fall were 120' long x 18' wide x 6' high. The area was bolted with 6' fully grouted bolts. Location of the fall was 1,600' outby the section dumping point.

LIFTING TIMBERS AND BEGAN EXPERIENCING ABDOMINAL PAIN. LOST TIME DID NOT BEGIN UNTIL 12/3/90. THE OPERATOR WAS INFORMED THAT THIS INJURY WAS DIAGONOSED AS A HERNIA ON 12/4/90.
TRAMMING SCOOP WHEN APPROACHED AN UNEVEN AREA OF HAULAGE ROAD. SCOOP BOUNCED-OPERATOR'S HEAD STRUCK SCOOP CANOPY. LOST TIME DID NOT BEGIN UNTIL 10/23/90. INJURY IS NOT OTHERWISE REPORTABLE.

THE INJURED WAS TRAMMING THE MINER BY RADIO REMOTE CONTROL TO THE FACE. HE HAD BEEN SETTING IN THE CROSSCUT WHEN THE SHUTTLE CAR EPERATOR WENT TO GET ON THE SHUTTLE CAR. THE INJURED MOVED OVER INTO THE CENTER OF THE ENTY. HIS LIGHT WAS FACING THE MINER AND THE SHUTTLE CAR OPERATOR COULD NOT SEE HIM. THE SHUTTLE CAR OPERATOR TRAMMED ONTO HIS LEG.
JOHN WAS LOADING ROOF BOLTS ON THE BOLTING MACHINE WHEN HE FELT A PULL IN HIS LOWER BACK. HE CONTINUED WORKING UNTIL 6:30 WHEN HE CAME OUTSIDE COMPLAINING ABOUT NUMBNESS IN HIS L EGS.
