ROOF BOLTING HAND WAS CAUGHT BETWEEN ROOF BOLTER HEAD.
EMP WAS PULLING CABLE WHEN HAND SLIPPED AND GLOVE GOT CAUGHT IN DRIVE SHAFT OF JEFFREY MINER TWO MIDDLE FINGERS RECEIVED LACERATIONS AND WERE MASHED
