A ROOF FALL OCCURRED IN AN INACTIVE SET OF ROOMS AT CROSSCUT NO.16 AT SPAD #607 & 608. THE FALL MEASURED 20 FEET WIDE, 70 FEET LONG AND 4.5 TO 5' HIGH. DOUBLE LOCK ROOF BOLTS 48" LONG WAS USED IN AREA.
EMPLOYEE WAS CLEANING ROADWAYS WITH A SCOOP AND CAUGHT HIS HEAD BETWEEN THE PANIC BAR AND THE TOP.
EMPLOYEE WAS LIFTING A ROCK FROM THE BELT FEEDER AND PULLED TENDONS IN ELBOW.
